Job summaryWe are looking for an RMN who is highly motivated, innovative, enthusiastic and passionate about patient centred care to work within the Bridgend south CMHT.
The team work with a variety of complex patients but are supportive. There will be occasional weekend working and relevant training needs can be supported
Main duties of the job- Undertake responsibility for the care co-ordination and case management of a defined caseload of clients with complex and challenging needs as defined within Parts 2 and 3 of The Mental Health (Wales) Measure 2010.
- Take a lead role in the care planning process, take on care co-ordination responsibilities and work within the Care Programme Approach ensuring that Service and CTMUHB policies are adhered to.
- Take part in the evaluation of the care of individuals at review meetings, case conferences, ward rounds etc.
- Provide specialist knowledge and advice to other agencies for the purposes of public protection.
- Attend relevant specialist assessment training and formulate to encourage safe and appropriate risk management .
- Attend all relevant mandatory training as and when required to do so.
- Undertake the necessary training and professional development to maintain the standards of professional practice and to meet the registration requirements of the NMC.

Person SpecificationAptitude and AbilitiesEssential- Ability to work in a challenging and changeable environment.
- Good communication and listening skills.
Desirable- Experience of working with patients with SMI.
Qualifications and/or KnowledgeEssential- Degree in Mental Health Nursing or equivalent skills, knowledge and experience plus Diploma level specialist training experience, short courses.
- Experience of working within a CMHT/ORT environment where experience and/or knowledge of Care Co-ordination and the All Wales Measure 2010.
Desirable- Experience in the use and knowledge of assessment of risk tools e.g. WARRN, HCR-20, and SVR-20.
- Welsh Language Speaking/Listening Skills at Level 3 or above
