Job summaryThe post holder will be required to identify and implement plans to meet the health needs for this very vulnerable group of children and young people. This will be achieved by improving the co-ordination, access and delivery of health care to children and young people in care both living in and out of area who are the responsibility of Derby City Local Authority.
As a Specialist Children in Care Nurse the post holder will provide expert clinical advice, complete statutory review health assessments, deliver health education and develop and deliver training programmes to a range of professionals.
Main duties of the jobTo undertake health assessments and formulating health care plans for children and young people in care living in Derby City and within a 20 miles radius, signposting and making referrals to other agencies/services as appropriate.
Act as an advocate for children and young people in care
To work with social care colleagues to ensure the health needs of children and young people, identified at health assessments are addressed appropriately with on-going follow up and review.
To work closely with the health visiting and school nurse service to ensure the health needs of children in care are being met.
To lead and implement health histories for young people leaving care
To contribute to and attend strategy meetings where there is safeguarding risk

Person SpecificationAttainment/QualificationsEssential- Qualified to diploma/degree level as Registered General Nurse, Registered Children's' Nurse or Registered Mental Health Nurse.
- Experience which includes working with children or young people with complex emotional needs.
- Post registration qualification in teaching and assessing or evidence of experience and competency.
Desirable- Post Registration qualification in Health Visiting / Community Practitioner Public Health Nursing or Specialist practitioner School Nursing
SkillsEssential- Interpersonal and communication skills.

- Assessment skills in child growth, development, emotional and physical well-being.
- Ability to travel the geographical area
Desirable- Experience using SystmOne Computer database
