Job summaryHASP PCN is a network of five GP practices across Hull. We are looking for a Community Paramedic, Nurse Practitioner or Advanced Nurse Practitioner to join our acute care team. The role is primarily to provide home visits to our housebound patients and support care home residents.
During periods of high demand there will also be a requirement to provide acute clinics at our practice sites.
Main duties of the jobThe Community Paramedic/NP/ANP will work as an integral member of the HASP PCN multidisciplinary team, delivering high-quality, patient-centred care within patients' homes, care homes, GP practices and community settings. The post holder will provide assessment, diagnosis, treatment and management of patients with acute, chronic and complex health needs, helping to reduce avoidable hospital admissions and improve access to primary care services.
About usThis role is based at Park Health Centre and forms part of our overlay PCN team. You will be working alongside a team of 20 people supporting our patients and wider care initiatives.
Job responsibilitiesThe role includes conducting home visits, running minor illness and urgent care clinics, supporting patients receiving palliative and end-of-life care, prescribing medications within scope of practice, and contributing to the delivery of Primary Care Network Directed Enhanced Service (PCN DES) requirements.

QualificationsEssential- Registered Paramedic or Nurse Practitioner or Advanced Nurse Practitioner.
- Independent Prescriber qualification.
- Evidence of ongoing CPD.
- Full UK driving licence and access to transport with appropriate vehicle insurance.
