Job responsibilitiesHASP PCN is a network of five GP practices in Hull serving a population of approx. 35,000 patients. The Primary Care Pharmacist (Independent Prescriber) will work as part of the multidisciplinary team (MDT) within the Primary Care Network to deliver high-quality, patient-centred clinical pharmacy services. The post holder will undertake structured medication reviews, support the management of long-term conditions, provide prescribing support, contribute to Quality and Outcomes Framework (QOF) achievement and assist in the delivery of PCN Directed Enhanced Service (DES) requirements.
The post holder will support medicines optimisation, improve patient outcomes, reduce health inequalities, and contribute to the delivery of national and local healthcare priorities.
Person SpecificationExperienceEssential- Experience working within primary care.
- Experience undertaking medication reviews.
- Experience managing long-term conditions.
- Experience of clinical prescribing.
- Experience working within multidisciplinary teams.
QualificationsEssential- Master's Degree in Pharmacy (MPharm) or equivalent.
- Registered Pharmacist with the General Pharmaceutical Council (GPhC).
- Independent Prescriber qualification.
- Completed CPPE pathway.
- Evidence of ongoing Continuing Professional Development (CPD).
