Job summary
We are seeking to recruit Social Workers to work in our service. The post-holder will work with a defined caseload of patients, most of whom will be in their own homes, providing direct, holistic assessment and care, in liaison with colleagues.

Social Workers within community mental health care for Older People are embedded within the Mental Health Teams which requires them to deliver the statutory duties under the Care Act 2014. Social Workers will be working with members of the multidisciplinary team including Mental Health Nurses, Doctors, Psychologists, Support Workers and Occupational Therapists.

Main duties of the job

• Provide direct patient care to a defined group of service users, most of whom will be in their own homes. This will include counselling, and assistance with achieving social inclusion. This may include patients detained in hospital as well as some subject to conditional discharge (such as S37/41)
• Assessment of risks
• Liaison with carers and other stakeholders
• Liaison with other members of the multi-disciplinary team
• Adherence to team and Trust policies, as well as national guidelines and legislation
• Support of learners
• Participation in a duty and triaging system
• Regular travel within Hackney and the City, and occasionally to neighbouring boroughs
• Working within the CPA framework, ensuring that all documentation is up to date and in line with NMC and Trust guidelines
• Participating in individual (and where available group) supervision and appraisal

• Evidence of continual professional development

• Evidence of portfolio.
• AMHP qualification

Experience
Essential

• At least 18 months' experience of working as a qualified Social Worker including relevant community experience.
• Experience of making full assessment under the Care Act and writing reports
• Experience of working as part of a multi-professional team
• Experience of working in a multi-racial inner city area.

Desirable

• Ability to engage with people holistically
• Ability to use Word and Excel
