Job summary
An exciting opportunity has arisen for a Advanced Clinical Practitioner (ACP) to join our Specialist Frailty Services based in the community across North East Essex.

We are seeking a dynamic ACP to join our multidisciplinary frailty team working within our expanding Frailty Rapid Assessment Service

Bringing Frailty and Older persons expertise you will play a key role in delivering an admission avoidance service with a focus on preserving function and independence, and supporting proactive patient care.

You will also be required to support other frailty services, including the Frailty Hospital @ Home service (Virtual Ward).

Working 9.5 hour shifts covering 7 days. Weekend working will be expected.

Banding and Progression:
o Band 7 if working towards a full MSc in Advanced Clinical Practice.
o Band 8A upon completion of a full ACP MSc, with clear progression to 8A once achieved, in line with Trust policy

Main duties of the job
Uses specialist knowledge and specific training to act autonomously in assessing, planning, and implementinga comprehensive management plan for undifferentiated presentations within their defined scope of practice.

Elicit a patient history appropriate to the clinical situation, which will demonstrate a holistic review of thepatient's physical and emotional wellbeing and needs.

Undertake physical examinations as indicated by the patient's condition; autonomously analysing complexclinical signs and investigation of results to consider differential diagnoses and diagnose conditions, initiatingtreatment as required.

Performs clinical procedures appropriate to the history and physical examination of the patient, includingfurther invasive testing and treatments requiring highly developed skills and precision, including (but not limited to catheterisation, cannulation, venepuncture, Prescribing and de-prescribing appropriately

Qualifications
Essential
  • Professional Registration with either NMC, GPhC or HCPC.
  • Possession of 1st degree in a clinical field e.g., Nursing, physiotherapy at a minimum 2:2 classification
  • Non-Medical Prescribing (v300) if appropriate to registered profession /role
  • Leadership/management course

Desirable
  • Continuing education
  • Working towards an MSc in Advanced Practice or recognised equivalent by HEE Centre Advancing Practice

Experience
Essential
  • Completed portfolio of supervised practice in advanced clinical practice.
  • Continued professional development following MSc completion.
  • Recent teaching, mentorship, and supervision of staff
  • Multi - professional working
  • Frailty or older persons understanding

Desirable
  • Evidence of clinical work in an autonomous role
  • Evidence of quality improvement methodology and audit experience
  • Evidence of involvement in change management
  • Working towards a full MSc in Advanced clinical practice
  • Frailty or Older persons Qualification
