Job summary
An exciting opportunity has come up to cover maternity with the Early Intervention in Psychosis team in Colchester for 12 months. We are looking for a dynamic person dedicated to providing timely, person-centred support to individuals experiencing first-episode psychosis.

Main duties of the job
Some of your typical duties as a Care Coordinator may include (but are not limited to):
  • Triaging and assessment of new referrals of clients, using specialist assessment tools such as the Comprehensive Assessment of At Risk Mental States (CAARMS)
  • Coordinating the care and assessment under the Care Programme Approach (CPA) of a designated caseload
  • Creating and continually updating person-centred care plans and building therapeutic relationships with your clients.
  • Continually assess risk and design personalised risk management plans
  • Facilitate referral to crisis teams or admission to hospital where necessary, plus providing presence and involvement in inpatient care plan and discharge planning.
  • If RMN, administer long-acting anti-psychotic depot injections in a community setting
  • Monitoring of medication adherence and any potential side-effects. Organising medical reviews when indicated.
  • Identify any possible safeguarding concerns and be responsible for the completion of safeguarding investigations.
  • Ensure that the requirements of the Mental Health Act 1983 are observed, adhered to and implemented
  • Support the promotion of carer and service user involvement within the service and for its provision plus ensuring carer's needs are identified and responded to.

Job responsibilities
The Early Intervention in Psychosis Service is a multi-professional community team providing a range of mental health and social care interventions to people between the ages of 14-65 years who experiencing their first episode of psychosis.The purpose of Early Intervention in Psychosis Service is to provide help for young people and adults experiencing a first episode of psychosis. This is in order to; Reduce distress Promote recovery Support carers and care networks Provide psycho-education and awareness Reduce stigma associated with psychosis and other co-morbid mental health conditions Prevention of relapse Monitor physical health and promote physical

Person Specification
Education
Essential
  • RMN, OT, or Social Worker relevant qualifications

Desirable
  • Additional relevant qualifications

Knowledge & Experience
Essential
  • Minimum 2 years post registration clinical experience (in any area)
  • Understanding of their responsibilities under the Care programme Approach
  • Understanding of their responsibilities under the Mental Health Act 1983 & Mental Capacity Act 2005

Desirable
  • Post-registration community mental health experience
  • Experience of carrying out the role of a care coordinator
  • Experience completing social circumstances reports, attending MHA tribunals, and monitoring and assessing S117 needs

Skills
Essential
  • Able to demonstrate Care, Compassion, Competence, Communication, Courage and Commitment
  • Effective verbal/written communication

Personal Qualities
Essential
  • Ensures that the organisational values of being open, compassionate and empowering are demonstrated in all areas of working life

  • Basic I.T and keyboard skills
  • Full UK driving license and access to a vehicle for purposes of carrying out role and responsibility
