Remote | Full-Time or Part-Time (22.5–37.5 Hours per Week) | £45,000 Pro Rata

We are seeking an experienced and highly motivated Occupational Therapist Case Manager to join our growing team. This is an exceptional opportunity for a skilled clinician looking to move into a rewarding and autonomous role where they can make a genuine difference every day.

Working remotely, you will manage a diverse caseload of clients with complex rehabilitation needs, coordinating tailored rehabilitation programmes and acting as the central point of contact for all parties involved in their recovery journey.

The Opportunity
As an Occupational Therapist Case Manager, you will utilise your clinical expertise, professional judgement and rehabilitation experience to deliver outstanding outcomes for individuals experiencing the physical, psychological, social and vocational effects of injury or illness.

Working within the framework of the Rehabilitation Code , you will coordinate rehabilitation programmes, monitor progress, liaise with key stakeholders and ensure individuals receive the right support at the right time.

This is a highly rewarding role that combines clinical knowledge, problem-solving, relationship management and rehabilitation planning.

Key Responsibilities
  • Undertake comprehensive rehabilitation assessments.
  • Develop, implement and coordinate individualised rehabilitation plans.
  • Apply advanced clinical reasoning to complex presentations.
  • Monitor treatment outcomes and adjust rehabilitation strategies accordingly.
  • Support individuals to maximise independence and quality of life.
  • Facilitate successful return-to-work and vocational rehabilitation programmes.
  • Liaise effectively with insurers, solicitors, employers, healthcare professionals and treatment providers.
  • Produce clear, concise and professional clinical reports.
  • Maintain accurate records in accordance with HCPC, RCOT and organisational standards.
  • Promote evidence-based practice and high-quality rehabilitation outcomes.

About You
You will be an experienced Occupational Therapist with a passion for rehabilitation and a commitment to delivering exceptional client-focused care.

Essential Requirements
  • Degree in Occupational Therapy.
  • Current HCPC registration.
  • Minimum of 5 years' post-qualification experience .
  • Full UK driving licence.
  • Enhanced DBS clearance.
  • Suitable ergonomic home working environment.
  • Strong IT skills, including experience using Microsoft 365 applications.
  • Broad clinical experience in areas such as:

Desirable Experience
  • Vocational Rehabilitation.
  • Return-to-Work programmes.
  • Supporting individuals to remain in or return to employment following illness or injury.
  • Acquired Brain Injury (ABI).
  • Spinal Injury Rehabilitation.
  • Paediatrics.
  • Hands and Plastics.
