Main duties of the jobThis exciting Registered Nurse Plus/HCP Plus post will provide an opportunity for individuals with excellent communication skills, clinical knowledge and leadership skills to act as team role models and be part of developing new ways of working, alongside quality improvement.This will include caseload management and undertaking assessments including MCA. Alongside MDT colleagues, providing excellent symptom control, psycho-social and spiritual support for patients who are under the care of the community teams supporting patients and their significant others through to end of life care.

Job responsibilities- As a Registered Nurse/Health Care Professional you will work within a multi-disciplinary team providing clinical care, navigation, coordination and support to hospice patients and their families
- Through holistic assessment provide information, advice, psychosocial and spiritual care to patients with palliative and end of life care needs, supporting their families and carers, liaising and referring on as necessary to appropriate health and social care professionals
- To work in partnership with those receiving care, helping them to access relevant health and social care, information and support when they need it. Support people to access, identify, appraise and interpret information, to enable them to determine their own future
- To maintain effective communication skills to deliver highly emotive information clearly, sensitively and unambiguously with staff, patients, families and professionals by face-to-face, telephone and/or electronic means
- To work collaboratively as part of the multi-disciplinary team (MDT)sharing the clinical caseload of patients/carers within the locality/in-patient unit for patients who have palliative care needs to ensure efficient and timely care is coordinated and delivered
- To work as part of the multi-disciplinary team, seeking advice from colleagues or/and clinical leadership team when required, in order to make sound decisions around palliative and end of life care
- To support management in training and mentoring a multidisciplinary workforce from novice to expert to work as one team
- To support any streamlined locality/IPU processes and services by embedding quality improvement methodology including clear outcome measures to produce efficient and highly performing community services
- To support the Hospice in building strong relationships with external partners and stakeholders and promote the good reputation of the Hospice e.g local trusts, ICB, health and social care partners, businesses and voluntary organisations
- To support and guide Health Care Assistants, Healthcare Support Workers and volunteers in their daily practice
Clinical Duties:- Clinical navigation, prioritisation and taking responsibility for the H@H caseload.
- Triage of all referrals into the locality team.
- Symptom assessment and management, in conjunction with the CNS Team.
- Leading on complex MCA and best interest decisions.
- Leading and mentoring on complex safeguarding issues.
- Assisting the Locality Care Team Manager in relation to concerns or complaints relating to the service as assigned, and identification of effective resolutions ensuring compliance with Hospice Policies.
- Promotion of health and safety within the Locality Care Team, ensuring staff are aware of current guidance and safe practice.
- Discussions on advance care planning to include ceilings of treatment, place of death and discussions around death and dying.
Management Responsibilities:- To promote the hospice values within the locality teams and act as a role model to other members of the Locality Team.
- To ensure effective communication channels are in place and all staff are engaged in working towards the one team ethos.
- To work with the Locality Care Team Manager, Deputy, CNS team and other members of the organisation to lead on key aspects of service improvement to achieve an effective and responsive service compliant with CQC requirements.
- To assist with monitoring and auditing standards of care to ensure that excellent care is being provided.
- Clinical leadership of the community team, including 1:1's, appraisals, role modelling and joint working.
- To work with the Locality Care Team Manager in the recruitment, selection, development and retention of staff, assisting with their induction when necessary.
Educational Responsibilities:- To act as a mentor to new members of the team, including newly qualified nurses, student nurses (to develop extended skills) and HCAs, coordinating induction and supporting in practice.
- To lead a specialist interest group - champions in a specialist area to work with the Education Team - to enhance and ensure evidence-based practice.
- To complete the orientation, induction and competencies for your role and assist other members of the wider team as directed.
Person SpecificationQualifications- Essential: Nurse Registration/HCPC registration, Evidence of continued professional development, Teaching and assessing qualification and/or extensive teaching experience
- Desirable: Advanced Communications Skills Training, Clinical Supervision Qualification, Post registration qualification in palliative care/cancer care/other medical specialty
Experience- Essential: Proven post registration experience, Experience in the delivery of end of life and palliative care or related disciplines, Relevant post registration experience at NHS Band 5 or above, in cancer, palliative or other relevant specialty, Minimum 2 years experience in palliative care/oncology or relevant discipline e.g. frailty, Demonstrable experience of successfully implementing new ways of working or adapting to change
- Desirable: Experience of holistic assessment and complex symptom management, Experience of multi-disciplinary team working, Experience of working unsupervised (Lone working), Experience of undertaking quality improvement projects and or research
Skills/Ability/Knowledge- Essential: Proven ability of successfully working with people and handling difficult and sometimes complex situations, Ability to work under pressure, manage own stress and emotional responses and be aware of stress in others, Ability to use a range of IT systems and software, including Word, Excel, PowerPoint, Office 365 and electronic patient record systems (such as SystmOne), as well as virtual systems e.g. Zoom, MS Teams, Ability to reflect and critically appraise own performance, Ability to motivate self and others and to lead by example, Able to effectively resolve professional and ethical issues, Ability to prioritise issues and tasks, Good oral/written communication and interpersonal skills, Able to lead a team of peers and junior members of staff to embrace new ideas and improvements in team performance, Ability to both participate and lead multidisciplinary team working, Understanding of the importance of data in driving service improvement
- Desirable: Ability to lead and or undertake a quality improvement project
