Job summary
This is an exciting opportunity to join the leadership team within our Intermediate Bed-Based Care Service. assessment, offering care and rehabilitation for patients and work with them to identify their own goals.

The successful postholder will provide professional leadership and contribute to the ongoing development of a responsive, high-quality service that can adapt to changing demands and support wider health and social care system priorities.

Working collaboratively with colleagues and partner organisations, you will play a key role in driving service development, delivering transformational change, and supporting system-wide initiatives that improve outcomes for our local community.

You will be expected to demonstrate strong, inclusive leadership, empowering staff and fostering a positive culture of collaboration, innovation, and continuous improvement.

This role is ideally suited to an experienced and motivated leader who is passionate about delivering high-quality patient care and improving services.

You will lead on quality improvement initiatives, support the development of innovative models of care, and ensure that patient experience, safety, and outcomes remain at the heart of service delivery.

This is a part time role working 30 hours per week.

Main duties of the job
As a valued member of the leadership team, you will have the opportunity to influence strategic and operational direction, support workforce development, and contribute to the continued success and evolution of the Intermediate Bed-Based Care Service, ensuring it remains responsive to the needs of patients, carers, and the wider healthcare system.

The postholder will lead by example, demonstrating First Communitys values and behaviours while fostering a culture of empowerment, innovation, collaboration and continuous professional development.

They will support, manage and develop staff, ensuring the workforce has the skills and capability required to meet organisational objectives and deliver excellent patient outcomes.

A key responsibility is leading the development and implementation of an Integrated Multidisciplinary Team (MDT) Model of Care, working closely with system partners to improve integration, patient flow and service effectiveness.

The role involves maintaining oversight of hospital discharge policies and transfer of care processes, ensuring compliance with national and local requirements and embedding these within operational practice.

Job responsibilities
This is an excellent opportunity to provide professional leadership, service development and support the introduction of new initiatives within a 28 bedded Intermediate Care ward.

The role is ideal for someone who is a strong team player who is able to foster strong inclusive leadership. We are looking for a practitioner who is highly motivated to lead on our continued commitment to providing innovative quality improvement programmes to ensure high standards of patient care.

This is a part time role working 30 hours per week.

Qualifications
Essential

• Professional NMC or HCPC Registration
• First level degree or equivalent
• Evidence of further relevant post registration training
• Leadership and Management training

Desirable

• MSC or MA or working towards

Knowledge
Essential

• Good working knowledge of Microsoft Office including Outlook, Excel, Word and PowerPoint

Experience
Essential

• Minimum 5 years post registration experience
• Experience of line management
• Experience of Audits and enhancing standards of patient care through service evaluations and quality improvement initiatives
