Job summary
Are you a motivated and enthusiastic District Nurse with a Specialist Practice Qualification looking to continue developing you career providing NHS care? We are looking for a highly skilled and enthusiastic Band 6 NMC registered District Nurse to join our Redhill Care Collaborative team. You will need to be forward thinking, motivated and able to work collaboratively with patients, their carers and health and social care colleagues. You need to have a flexible approach to working some weekends and bank holidays as the teams provide a 365 day service. This role is part-time, 30 hours per week across a 7 day a week service, with an expectation to travel across the geographical patch.

Main duties of the job
Working as part of a team, you will be supported by the Team Lead to be responsible for the continued management of a caseload of patients. You will be working with other health care professionals across the neighbourhood, providing high standards of care to people in their own homes who have a wide range of nursing needs; aiming to avoid hospital admission, promoting self-care and enabling people to be cared for in their preferred place. As a District Nurse you will have line management responsibilities and will support the Team Lead with the teams ongoing development and well-being. Due to the nature of the role, it is essential that you can drive, have a car suitable for business purposes, and a valid driving licence.

Job responsibilities
The District Nurse is responsible for the management and delivery of a high standard of cost-effective nursing care in the community. You will be responsible for the continued management of a caseload of patients, working within a neighbourhood providing care closer to home aiming to avoid hospital admission. You will ensure that patients receive safe, individualised, and effective standards of care by assessing, planning, implementing, and evaluating episodes of care. You will work as part of a highly skilled team, working closely with patients and carers, linking with acute hospitals and primary care, actively encouraging effective communication at all times with all relevant agencies. You will promote and develop evidence-based practice in line with current policy and procedures and NMC code of conduct. You will line manage staff and be responsible for the development and competencies of staff within their team. The service currently operates 365 days a year between 8am and 6pm and you will be required to work shift patterns between these times. Experience
  • Evidence of post registration experience and study at level 6
  • Evidence of professional development with a broad range of clinical skills
  • Evidence of team management and MDT working
  • Experience of initiating and participating in quality improvement

Qualifications
  • Registered Nurse (RN1)
  • Specialist Practice Qualification in District Nursing

  • Good IT skills

Knowledge
  • Good understanding of current nursing workforce issues
