An exciting opportunity has arisen for a Practice Nurse to join our well-established and supportive nursing team at Forum Family Practice, Cramlington.

We are looking for an enthusiastic and motivated Practice Nurse with a good foundation in primary care nursing, who is keen to contribute to the delivery of high-quality patient care.

Ideally, the successful candidate will have experience and training in the following areas:

• Cervical cytology.
• Childhood and adult immunisations.
• Travel vaccinations and advice.
• Long-term condition management, including COPD, asthma and diabetes.
• Contraception initiation and follow-up.

We appreciate that not all nurses will have training or experience in every area listed above. We would therefore encourage applications from nurses who have some of these skills and are enthusiastic about developing their knowledge and competencies further. Main duties of the job
The successful candidate will join our well-established nursing team, delivering high-quality, patient-centered care across a range of primary care services.

• Deliver a high standard of clinical care, supporting the provision of core and enhanced nursing services, including chronic disease management, health promotion, immunisations and screening programmes.
• Undertake cervical cytology, childhood and adult immunisations, and travel vaccinations.
• Support the management of patients with long-term conditions, including diabetes, COPD, asthma, heart failure and stroke.
• Provide sexual health and contraception advice, including contraception initiation and follow-up where appropriately trained.

Main Duties and Responsibilities / Clinical Responsibilities

• Provide high-quality nursing care, including assessment, planning, implementation and evaluation of patient care.
• Undertake cervical cytology in accordance with NHS screening guidance.
• Deliver childhood and adult immunisation programmes, including seasonal vaccination campaigns.
• Provide travel health advice, risk assessments and travel vaccinations where appropriately trained.
• Undertake health promotion, lifestyle advice and disease prevention interventions.
• Provide sexual health services, including contraception reviews, initiation and follow-up where appropriately trained.

Long-Term Condition Management
Undertake structured reviews and support patients in managing their long-term conditions, including:

• Diabetes.
• Asthma.
• COPD.
• Cardiovascular disease and stroke prevention.
• Heart failure.
• Hypertension and other long-term conditions as appropriate.

Responsibilities include monitoring disease progression, reviewing symptoms, supporting treatment plans, providing patient education and escalating clinical concerns to the appropriate clinician.

The postholder should contribute to achieving relevant practice and NHS quality indicators, including QOF and locally commissioned services.

Professional Responsibilities

• Work autonomously within the limits of individual competence and professional accountability.
• Maintain accurate, timely and confidential clinical records using the practice clinical system.
• Follow current evidence-based guidance, clinical protocols and practice policies.
• Ensure medicines and vaccines are administered safely and in accordance with appropriate authorisation, including Patient Group Directions (PGDs) or Patient Specific Directions (PSDs), where applicable.
• Recognise and respond appropriately to clinical deterioration and emergency situations.
• Maintain infection prevention and control standards.
• Recognise safeguarding concerns involving children and vulnerable adults and follow relevant reporting procedures.
• Participate in clinical audits, quality improvement initiatives and service development.
• Identify clinical risks and report incidents, near misses and significant events appropriately.

Team Working and Education

• Work collaboratively with GPs, ANPs, Practice Nurses, HCAs and the wider multidisciplinary team.
• Support the development of a positive learning environment within the practice.
• Participate in the supervision and education of student nurses and other team members, where appropriately trained.
• Share knowledge and experience to support the development of the nursing team.
• Attend nursing meetings, practice meetings and relevant multidisciplinary discussions.
• Contribute to the development and implementation of clinical policies, protocols and new services.

Organisation and Administration

• Manage and prioritise an individual clinical workload effectively.
• Ensure appointments and clinical activities are managed safely and efficiently.
• Maintain accurate documentation and appropriate clinical coding.
• Support the effective management of clinical stock, vaccines, equipment and medical supplies.
• Ensure clinical equipment is used safely and report any faults or concerns.
• Maintain patient confidentiality and comply with information governance requirements.

• Contraception and sexual health training
