Paediatric Diabetes Family Support Worker

Band 4 | 18.75 hours per week | Permanent | Frimley Park Hospital

Are you passionate about supporting children, young people and their families? Do you have the empathy, communication skills and practical approach to help families navigate the day-to-day challenges of living with diabetes?

We are looking for a Paediatric Diabetes Family Support Worker to join our friendly and dedicated paediatric diabetes multidisciplinary team at Frimley Park Hospital.

This is a rewarding opportunity to provide practical, emotional and social support to children and young people living with diabetes, helping them and their families build confidence and develop the skills they need to manage diabetes in everyday life.

What you'll do
Working alongside the paediatric diabetes multidisciplinary team, you will:

• Provide practical, emotional and social support to children, young people and their families.
• Help families build confidence in managing the day-to-day challenges of diabetes.
• Support children and families to engage positively with healthcare services.
• Identify social, emotional, educational or practical difficulties that may impact a child's wellbeing or diabetes management.
• Contribute to a positive, supportive experience for families throughout their diabetes journey

Main duties of the job

• Provide practical and emotional support to children and young people with diabetes and their parents or carers.
• Build positive, trusting relationships with children, young people, families, and members of the multidisciplinary team.
• Help families understand information provided by the diabetes team and reinforce agreed advice in an accessible and age-appropriate way.
• Support families with the day-to-day management of diabetes, within the boundaries of the role and under appropriate clinical guidance.
• Encourage children and young people to develop confidence and independence in managing their diabetes as they become older.
• Identify barriers that may affect diabetes management, such as family circumstances, emotional wellbeing, school issues, financial difficulties, or difficulties engaging with services.
• Signpost or refer families to appropriate community, social care, education, psychological, and voluntary-sector services when required.
• Work collaboratively with schools and education staff to support children with diabetes and promote appropriate support within the school environment.
• Support families to attend appointments and engage with the paediatric diabetes service

Qualifications
Essential

• Successful completion of Foundation Degree equivalent to Diploma level.
• Evidence of continued professional development documented through Trust PDR/KSF/Supervision processes and personal portfolio.

Desirable

• Evidence of recent update in safeguarding Children

Experience
Essential

• Experience of working with children and young people with diabetes.
• Experience of working within a multi-disciplinary team.
• Experience of working in a community role.
• Experience of teaching.

Skills & Knowledge
Essential

• Commitment to undertake further training and desire to develop new skills.
• Ability to motivate others.
• Good organisation skills.
• Evidence of effective interpersonal communication skills.
• Ability to problem solve.

Desirable

• Clinical skills e.g. measurement and documentation of height, weight, blood pressure, urinalysis and HbA1c.
• Full understanding/application of relevant clinical practice/standards/audit within identified clinical area.
• Good understanding of diabetes issues - including medication management and user involvement.

Special Requirements
Essential

• Ability to travel independently e.g. full driving licence.
• Ability to work flexibly to meet the needs of the service in conjunction with other members of the team.
