GDOC has an exciting new opportunity for a full-time, enthusiastic and experienced Clinical Pharmacist to work for Dean Medical Practice - Coleford.

The successful candidate will become an integral part of the general practice team, working either at Coleford Health Centre, Brunston Surgery, or Lydbrook Health Centre, as well as being active within a network of wider community groups and their multi-disciplinary teams. They will work with existing groups and work collaboratively with health care professionals internally and externally.

The role is pivotal to improving the quality of care for patients and operational efficiencies within Practices and therefore requires motivation and passion to deliver excellent service within general practice.

Annual Salary: £54,600 WTE

Main duties of the job
To improve patients health outcomes and the efficiency of the practice team by providing direct, accessible and timely medicines expertise, including identifying areas for improvement and initiating change.

To be the primary contact within the practices for queries relating to medicines management.

To focus on individual patient care by addressing immediate and future needs through personalised care planning.

Proactively to transfer, from other clinical staff, workload relating to medicines optimisation to improve patient care and safety.

To demonstrate the effectiveness of the practices and PCN, and the CP role through audit and data analysis.

Job responsibilities
Duties Specific to Role

• Ensure medication management systems are safe and effective, liaising with GPs and managers as required.
• Ensure that proper processes are in place for medication review, and undertaking medication review.
• Undertake structured medicine reviews
• Implement and embed a robust repeat prescribing system
• Review and implement safe prescribing systems for vulnerable patients, improving compliance
• Review and implement safe prescribing systems for patients suffering from drug and alcohol dependencies
• Encourage cost-effective prescribing within the practice
• Review all safety alerts ensuring the required actions are taken
• Reconcile medicines following patients discharge from hospital or clinic appointments, or any other contact with a third party provider,
• Review medications for newly registered patients
• Review clinical reporting in SystmOne relevant to medicines management, and ensure Practice is meeting its contractual requirements and CQC standards, and achieving KPIs (e.g. QOF)
• Design and run clinical reports & audits to support best practice, and implement any identified changes required.
• Implement medication management policies and processes of the Primary Care Network
• Liaise with pharmacy team colleagues in the Primary Care Network, and support the implementation of best practice in the PCN.
• Liaise with ICB pharmacy team colleagues.
• Ensure (in liaison with the lead GP and practice manager) that all contractual requirements relating to medication are met.
• Support and advise pharmacy team colleagues, including trainees and dispensary staff.
• Act as line manager to pharmacy team colleagues, including trainees.
• Support the professional development of pharmacy team colleagues, including trainees.

The multidisciplinary team will work together to

• Improve hypertension detection rates by overseeing the introduction of digital tools/pods in practice waiting rooms that link patient data to the medical records. These digital tools/pods will record blood pressure, heart rate and BMI in the waiting rooms, at the convenience of registered patients. Protocols will be developed that will ensure clinical safety and governance and enable abnormal readings to be actioned appropriately; patients requiring intervention will be triaged and reviewed by the most appropriate clinician, whether this be a GP or a pharmacist.
• Develop and utilise a risk-stratified approach to conducting the hypertension management reviews on the hypertension register at the practices. Population health data and evidence-based guidelines would inform the development of a red, amber, green (RAG) clinical risk-stratified approach to hypertension management.
• Improve the annual attainment of blood pressure readings within the target range set by national guidelines.
• Improve the annual attainment of patients receiving lipid therapy for primary prevention of cardiovascular disease.
• Review the management of patients across with coronary heart disease and conduct medication reviews to ensure medical therapy and lifestyle interventions have been maximised according to need.
• Identify those requiring additional support to attend their appointments and provide accessible appointments in person, via telephone or via appropriate digital platforms and utilise communication methods appropriate for the needs of the patient.

Non-medical prescribing/independent prescribing
As a qualified non-medical prescriber (NMP)/independent prescriber (IP), prescribing will be undertaken within the scope of practice of the prescriber and in accordance with their professional code of conduct as set out by their own regulatory bodies and organisational NMP policy.
