Job summary
An exciting opportunity has become available for a skilled, knowledgeable, and forward-thinking Diabetes Specialist Nurse to join our Community Diabetes Service, covering the Gloucester locality.

Based at Southgate Moorings, you will be part of a dedicated community-based diabetes team working within primary care. The role involves close collaboration with healthcare professionals across General Practice and Community Hospitals to support and enhance diabetes care delivery.

We are seeking a motivated and enthusiastic registered nurse with a recognised diabetes qualification and recent diabetes experience. Experience working within primary care and possession of a Non-Medical Independent Prescribing qualification would be advantageous.

The successful candidate will be confident working independently, possess excellent communication and interpersonal skills, and be able to travel throughout the locality. A full driving licence and access to a vehicle are therefore essential.

Comprehensive induction and ongoing professional development will be provided, alongside mentorship from our experienced Community Diabetes Team.

Main duties of the job
The role involves managing a diverse community-based caseload, predominantly supporting patients with Type 2 diabetes. Care is delivered across a variety of settings, including community hospitals, GP practices, outpatient clinics, and patients' homes. We also provide mentored/virtual clinics with primary care staff. Patient education and supporting self-management are key components of the position, alongside providing education and training to healthcare professionals.

Our Community Diabetes Service is innovative and progressive, continually evaluating and developing practice through service improvement, audit, and workforce development initiatives. We are looking for someone who is adaptable, embraces change, and is keen to contribute to the ongoing evolution of the service.

We have a skilled and dedicated workforce of over 5000 colleagues working in a diverse range of services over 55 sites and within people's homes. Qualifications
Essential
  • Current NMC registration
  • Level 6 diabetes

Desirable
  • Post graduate diabetes learning

Experience
Essential
  • Experience in assessing, planning , implementing care for patients with Type 2 Diabetes
  • Evidence of collaborative MDT working
  • Understanding of local and national guidelines for diabetes
  • Experience of audit
  • Experience in helping patients to self manage
  • Full UK drivers license
  • I.T literate

Desirable
  • Experience within a specialist community diabetes service
  • Experience in insulin initiation and continuous glucose monitoring
  • Independent non medical prescribing
  • Experience of training health care professionals
