Job summary
The Home Engagement and Rehabilitation Team (HEART) was set up with the aim of reducing reliance on mental health inpatient units, by providing community rehabilitation support to mental health service users with complex support needs.

The team provides intensive support to these service users throughout the hospital discharge process and beyond, to help those who would have otherwise struggled to move back into the community.

HEART is an integrated team, comprised of a variety of professionals who offer effective, goal orientated, person-centred care daily.

HEART currently operates in Manchester and Salford, with the aim of rolling out to other areas of Greater Manchester.

You will act as senior pharmacist for the Rehab Care Group and lead and develop the HEART team pharmacy service, combining specialist clinical practice with service development, medicines governance, financial management, education and influencing prescribing.

You will take professional responsibility for pharmacy's contribution to the HEART team, and rehab service setting priorities, improving services and ensuring medicines are used safely, effectively and within available resources.

Pharmacist independent prescribing responsibilities support this practice. You will be expected to contribute to planning, education, research, and participate in clinical audit within own area of practice. e.g. provision of education and training, procedures, policies and guidelines, and medicine usage information.

Main duties of the job

• Lead, deliver, develop and evaluate specialist clinical mental health pharmacy services within the HEART team
• Act as senior pharmacist for Rehab Care group, providing support for pharmacy team within rehab and attending care group Medicines Management Meeting
• Provide expert pharmaceutical advice and liaison to the HEART team, GPs, practice pharmacists, other healthcare professionals (HCPs), and patients and carers.
• Act as an independent prescriber of medication, ensuring that drug therapy is evidence based, safe, cost effective and appropriate to the individual patient.
• Provide training to the HEART team, GPs and other HCPs to ensure competency in prescribing, administration and monitoring of medication
• Analyse prescribing data to support cost-effective prescribing, budget management, and service improvement initiatives
• Review medication-related incidents, support pharmacovigilance, and ensure compliance with national and local prescribing guidance, including safe prescribing procedures and adherence local, Trust and National Policies
• Develop, review, update organisational policies, procedures, and guidance to maintain high standards of medicines management governance
• Work with clinical and operational colleagues, external healthcare providers, to promote best practices and patient-centred care
• Implement and support digital tools for medication management, including electronic prescribing and remote medication reviews
• Take part in the pharmacy on call service

Person Specification
Essential

• Current GPhC registration
• Post Graduate Certificate in Clinical Pharmacy or equivalent
• Post Graduate qualification in Psychiatric Pharmacy
• Independent Prescribing Qualification or working towards one
• Experience of working as a pharmacist in Psychiatry
• Experience of Line Managing and Supervising Staff

• Experience in Medicines Safety
• Experience of designing and delivering training to healthcare staff
• Evidence of research work
• Evidence of medicines management/ policy/procedure review
• Experience of working across primary and secondary care
