Job summary
Guy's and St Thomas' NHS Foundation Trust is seeking a clinically skilled pharmacist to support the Care Homes & Frailty Pharmacy Service across Lambeth and Southwark. The post holder will contribute to the safe and effective use of medicines for older adults living with frailty, providing clinical expertise across care-home and community settings.

The role centres on improving medicines optimisation processes, strengthening multidisciplinary working, and supporting consistent, person-centred care across the frailty pathway. The post holder will contribute to service development through audit, guideline work and quality-improvement activity, ensuring alignment with local and national priorities.

Regular mentorship and clinical supervision will be provided by senior pharmacists within the service, supporting the development of specialist knowledge in frailty therapeutics and progression towards senior practice. The position requires a motivated clinician with strong communication skills, sound clinical judgement and the ability to work effectively across organisational boundaries.

Please note: we are advertising for 3 positions in total. 2 x Band 8a positions at 0.6WTE and 1 x Band 7 position at 1.0WTE. Individuals can apply for both vacancies if they wish.

Main duties of the job
As a Care Homes and Frailty Pharmacist, you will provide clinical pharmacy input across care homes, community settings and primary care interfaces in Lambeth and Southwark. You will undertake medicines reconciliation, structured medication reviews and targeted optimisation interventions for older adults living with frailty.

You will work with pharmacy technicians, care-home staff, GPs, geriatricians and adult social care teams to address medicines-related risks, support safe prescribing and deprescribing, and ensure effective communication of pharmaceutical care plans. You will contribute to improving medicines systems within care homes, including reviewing processes and supporting implementation of best practice.

Your responsibilities will include participating in audit and quality-improvement activity, contributing to guideline and SOP development, and delivering training to registered and non-registered staff. You will take part in MDT discussions and case reviews to support coordinated, person-centred care.

Qualifications
Essential
  • Registered member of GPhC
  • Postgraduate Certificate or Diploma or equivalent experience

Desirable
  • Qualified non-medical prescriber
  • Specialist training, qualification or education in the care of older people

Experience
Essential
  • Working within multidisciplinary teams across primary, community or social care
  • Undertaking medication reviews in older people including adherence support and deprescribing
  • Delivering direct pharmaceutical care to patients with long term conditions and frailty
  • Delivering training to care home staff or community teams or non-pharmacy teams
  • Developing and undertaking audits

Desirable
  • Experience working across integrated care settings (PCNs, GP practices, community services and social care)
  • Experience managing patients independently (e.g., polypharmacy clinics, anticoagulation clinics, frailty pathways)

Knowledge
Essential
  • Demonstrable knowledge, skills and ability to undertake clinical screening and medicines reconciliation
  • Demonstrable experience of clinically influencing at senior level e.g. ward rounds or MDT meetings
  • Understands primary and secondary care working and organisational structures in relation to the ageing population
  • Understands the effects of ageing on drug handling and medicines use
  • Understands pharmacy and medicines risks in older people living with frailty, polypharmacy, multimorbidity and the role of pharmacy to mitigate those risks

Desirable
  • Understands national policies and the context around managing long term conditions, case management and reducing unnecessary hospital admissions in the NHS and their implication for medicines management

Skills
Essential
  • Ability to provide high-quality clinical advice and communicate complex pharmacy issues that may be frequently challenged
  • Willingness to tackle difficult issues as well as challenge practices and systems that lead to sub-optimal use of medicines
  • Highly developed communication, interpersonal, negotiating and influencing skills
  • Able to recognise limitations and scope of their practice and know when to escalate matters appropriately
  • Ability to make important decisions in pressured situations.
