Job summary
Are you a Children's Nurse passionate that no child should be in hospital if care can be provided closer to home and do you want to build on your clinical skills in the community? If so, HIOWH are looking for two Acute Children's community Nurses to join our Children's outreach assessment and support team (COAST).

The successful candidate will be an integral part of the team based at the Battenburg Child Development Centre, providing assessment and support to Babies, Children and young people aged 0-16 across a range of acute pathways, where no two days are the same.

There's lots of training available to gain new skills, develop and enhance existing skills.

Main duties of the job
  • To support the Team Leader by carrying out specific health care assessments and/or interventions
  • To carry out initial assessment and triage of patients with short term health care needs by telephone, at clinical settings or in patient's homes under the supervision of a Team Leader
  • To provide continuity of care for children on caseload and maintain contemporaneous records accordingly.
  • To discharge patients according to pathways of care, under supervision of Team Leader.
  • May carry out initial home assessments with guidance from the Team Leader to plan, implement and evaluate programmes of care.
  • To work in partnership with families and the multi-disciplinary team to provide care in appropriate settings to maximise life opportunities for the child or young person.
  • Report any changes in the child's condition to the Team Leader and medical colleagues and ensure all records are maintained in accordance with Trust policies.
  • Participate in the provision, storage and administration of medicines including intravenous therapy in accordance with Service/Trust guidelines and policies and in partnership with pharmacy colleagues.
  • Participate in the collection of data as required for evaluation of the service and take part in audit as required by the Trust.
  • Undertake specific clinical tasks and skills for which the nurse has received training and is deemed competent in all clinical settings.

Qualifications
Essential
  • RN Child
  • Evidence of ongoing development
  • Teaching/Mentorship qualification or willingness to undertake.

Experience
Essential
  • Experience of nursing children with acute/short term health needs.
  • Experience of interagency working.
  • Experience of effective working collaboratively with professionals from other disciplines.

Desirable
  • Experience of nursing children with long term conditions, complex needs and disabilities.
  • Experience of planning and delivering training.
