Job summaryWe are currently recruiting for an enthusiastic & motivated Band 5 Occupational Therapist to join our Integrated Community Team. You'll be joining a friendly and supportive team known for its collaborative approach and commitment to delivering high-quality, person-centred rehabilitation and enabling individuals to achieve their goals within their own homes and communities.
As a Community Occupational Therapist, you will work as part of a multidisciplinary team to assess, treat, and support adults with a wide range of health needs. You will provide holistic assessment and intervention, including rehabilitation, falls prevention, moving and handling assessments, postural management, equipment provision, and environmental adaptations to promote independence, safety, and wellbeing.
You will manage a varied caseload and work closely with colleagues across health and social care to support people to remain at home, prevent avoidable deterioration, and facilitate recovery following illness or hospital admission.

Main duties of the job- To provide specialist therapy skills within the Community Care Team.
- To assess and treat own caseload, which may include complex presentations and to maintain own records as an autonomous practitioner.
- To assist senior staff in the education and supervision of junior colleagues, assistants and students.
- To assist senior staff in daily management of the team and provision of therapy service, deputising for the team leader in their absence.
- To assist senior staff in development and maintenance of an efficient and effective therapy service.
- To contribute to the inter-disciplinary working within the Community therapy team and Locality by teaching, learning & practicing inter-disciplinary skills & techniques as defined by an agreed competency framework or training scheme.

- FULL UK DRIVING LICENCE is required at time of applying for role.
