Job summary
We are seeking a motivated, forward-thinking Band 5 Occupational Therapist to join our dynamic Hospital at Home service. This post is open to ambitious OT's (including newly qualified) who wish to be supported to progress to Senior OT level. It is an exciting opportunity to be part of an innovative, fast-paced team delivering hospital-level care in patients' own homes.

You will play a key role in providing rapid, holistic assessments and interventions for adults with acute illness, frailty and exacerbations of long-term conditions. Working autonomously, you will deliver specialist OT to support admission avoidance, early discharge and the promotion of independence and quality of life.

As part of a highly skilled MDT, you will work closely with Consultant Practitioners, ACPs, GPs, community services, ambulance crews and social care partners to provide coordinated, person-centred care. You will contribute to complex case management and lead therapy input, bringing your clinical expertise to shape patient care.

You will also support the development of others through supervision, teaching and role modelling best practice. Working within a responsive and supportive team, you will demonstrate flexibility and teamwork in meeting the needs of a rapidly changing caseload, ensuring timely and compassionate care for patients.

Main duties of the job
  • Deliver rapid, specialist occupational therapy assessment and intervention for adults with frailty, acute illness and complex health needs
  • Develop and implement evidence-based, person-centred care plans to support admission avoidance and recovery at home
  • Manage a varied and complex caseload, promoting independence, function and safety in patients' own environments
  • Work autonomously within a supportive multidisciplinary team, responding promptly to referrals and changes in patient need
  • Collaborate closely with GPs, ACPs, Consultant Practitioners, community teams and social care to provide coordinated care
  • Communicate effectively with patients, families and carers to enable shared decision-making and goal setting
  • Contribute to safe discharge planning and seamless patient pathways across services
  • Provide supervision, support and informal teaching to junior staff, students and unregistered colleagues
  • Maintain high standards of clinical documentation, professional practice and patient safety in line with HCPC requirements
  • Actively contribute to service development, quality improvement and evidence-based practice initiatives
  • Travel to patients' homes and community settings, working in a fast-paced, responsive service

Person Specification
Qualifications
Essential
  • Qualified Occupational Therapist
  • HCPC registration

Experience
Essential
  • Experience working with people living with frailty
  • Good knowledge of health and safety and risk management
  • Computer literate including emails and spreadsheets.
  • Effective written and verbal communication skills

Desirable
  • Experience working in services delivering admission avoidance for acutely unwell adults
  • Relevant clinical setting experience achieved as a postgraduate
