Job summary
An exciting opportunity has arisen for a Mental Health Nurse/Occupational Therapist/Social Worker working within the Adult Mental Health team at Parkway Centre, Havant. You will have the opportunity to be part of a forward thinking team and provide support to people with severe and enduring mental health needs, who require community support.

The work is varied and interesting and there is a strong team approach ensuring good support for the successful applicant. The role requires lone working, use of initiative and a commitment to team working and service user empowerment. You will need to be able to work flexibly and to demonstrate solid experience of the CPA process, risk assessment, and the recovery model.

Valid driving license and access to suitable vehicle for work purposes is essential.

At present, the Trust is unable to offer Skilled Worker visa sponsorship for this post. Applicants must have the right to work in the UK at the time of application.

Main duties of the job
To provide recovery based community mental health services within a multidisciplinary team to adult individuals with severe and enduring mental health needs referred to the Community Mental Health Team.

This role, in collaboration with referred individuals and where appropriate family or carers, will coordinate and provide recovery focussed assessments, risk formulations, care planning, crisis and contingency planning, evidence based interventions, response, reviews, reports, discharge planning and attendance at appropriate meetings.

Our neurological services offer rehabilitation and treatment for conditions including Multiple Sclerosis, Parkinson's Disease, Motor Neurone Disease, Head Injury, Cerebral Palsy and Stroke. Across Hampshire, our community hospitals provide inpatient rehabilitation as a step down from acute care, and our dedicated teams also staff Treetops Sexual Assault Referral Centre in Portsmouth, offering expert, compassionate support.
