Hartlepool and Stockton Health are advertising for Dovecot Surgery.

We are seeking an experienced and motivated Nurse Practitioner to provide autonomous clinical care to patients across a broad range of acute and chronic conditions; a respiratory background would be preferred. This is an exciting opportunity to become part of a supportive and friendly team.

Job responsibilities

• Deliver high-quality clinical care, including assessment, diagnosis, and treatment of patients
• Diagnose and manage acute and long-term conditions.
• Independently prescribe medications within your scope of practice.
• Order, interpret and act upon diagnostic tests and investigations.
• Support long-term condition management and proactive patient care, providing health advice, disease prevention and lifestyle advice.
• Refer patients to secondary care and specialist services when required.
• Maintain accurate and timely patient records.
• Work collaboratively with GPs, practice nurses, ARRS staff, and the wider MDT
• Contribute to service development and quality improvement initiatives

Person Specification
Experience
Essential

• Strong clinical assessment and decision-making skills
• Ability to work autonomously and as part of a team
• Excellent communication and patient engagement skills
• A proactive and compassionate approach to patient care
• Ability to work autonomously and as part of a team.

Desirable

• Experience managing chronic disease clinics.
• Lead on respiratory care, including COPD and asthma reviews, management, and optimisation in line with best practice
• Knowledge of UK primary care systems and pathways.
• Experience using SystmOne.

Qualifications
Essential

• Registered Nurse with current registration with relevant professional body.
• Qualified Nurse Practitioner with relevant prescribing qualification or working towards
