Package Description: Make a real difference where it matters most. Are you a passionate Sexual Health Nurse ready to take your impact beyond the clinic walls? At HCRG Care Group, we're redefining community sexual health - reaching people who need us most and tackling health inequalities head-on. This is more than a nursing role. It's your opportunity to shape services, influence outcomes, and deliver care in a way that truly changes lives. You will receive a Band 6 AfC salary with NHS pension and Agenda for Change terms and conditions. As an Outreach Nurse Specialist, you'll deliver high-quality sexual health and contraception services directly into communities, focusing on people who may not otherwise access care.
Main duties of the job
As a Band 6 Sexual Health Outreach Nurse, you'll deliver high-quality sexual health and contraception services directly into communities, focusing on people who may not otherwise access care. As an Outreach Nurse Specialist, you will provide expert, dual-trained sexual health and contraception care. You'll lead and deliver outreach to marginalised and underserved groups, identify unmet needs, and design targeted interventions. Your role also involves working closely with partners such as mental health services and homelessness teams. You will ensure smooth pathways between outreach and clinical services for complex cases. You'll play a key role in service development, data-driven planning, supporting colleagues, and promoting continuous improvement.
