Virtual Ward Team Sister/Charge NurseThe closing date is 16 August 2026Job IntroductionAs a Band 6 Virtual Ward Team Sister/Charge Nurse, you will join our innovative Virtual Ward Service, helping patients receive high-quality care in the place they call home. This is an exciting opportunity for an experienced nurse who is passionate about delivering proactive, patient-centred care while taking on leadership responsibilities within a supportive multidisciplinary team. You will manage a caseload of patients with complex health needs, frailty and long-term conditions, helping to prevent unnecessary hospital admissions and supporting patients to remain independent for longer. Working closely with GPs, community services, specialist teams and social care colleagues, you will play a key role in coordinating care, improving patient outcomes and shaping the future of Virtual Ward services.
