Job summary
Health Partners At Violet Melchett are looking for a practice nurse for our busy 11,000 patient GP practice. Alongside traditional GP services we offer a range of community services and activities so your job will be busy and varied. You will be based at our premises just off the Kings Road, Chelsea SW3.

Main duties of the job

• Core nursing services to provide all services
• Health promotion activities, such as smoking cessation, weight management, dietary and exercise advice
• Sexual health and contraception
• Cervical cytology
• Childhood immunisations
• Travel health
• Blood taking
• Ear syringing
• Assess and care for patients presenting with wounds
• Removal of all types of skin closure materials
• See patients for urgent and opportunistic nursing care
• New patient checks
• NHS health checks (vascular risk)
• Demonstrate and teach patients, as appropriate, principles of care and procedures, working towards their independence and self-care
• Ambulatory BP

Administer drugs and vaccines by any route, according to NMC, practice and, where appropriate the NHS England guidelines.

Extended role of nurses to offer a range of services from this list.

Long Term Conditions management Including annual reviews:

• Cardio-renal metabolic, asthma and COPD, hypertension and heart disease
• Minor ailments and ECGs

About us
Health Partners at Violet Melchett is a GP Practice which is based in the Violet Melchett Health and Wellbeing Hub in Chelsea and to a satellite site in Earls Court Health and Wellbeing Hub. We are at the centre of a new and exciting integrated health and wellbeing model to serve the needs of the local community covering the Brompton PCN area in the south of Kensington and Chelsea.

Along with our GP clinical services, we operate alongside a range of integrated services including community nursing and mental health services, extended Access, social prescribing link workers, navigators, and volunteers. The services are delivered in partnership with many different providers of healthcare, including the local NHS community health trust, the NHS mental health trust, General Practice, local authority services, and services and activities provided by the third sector and local communities. In addition, the Violet Melchett Health and Wellbeing Hub offers flexible space for local people, community groups and the voluntary sector.

Essential experience

• 2 years experience working in primary care

Essential qualifications and skills

• Knowledge of needs of patients with long-term conditions, selected from a range including: diabetes, asthma, COPD, hypertension, heart disease, anti-coagulation therapy
• Aware of accountability of own role and other local and national roles in a nurse led service
• Knowledge of health promotion strategies
• Awareness of clinical governance issues in primary care
• Prescribing according to NMC guidelines
• Knowledge of patient group directions and associated policy
• Clinical skills: cervical cytology, immunisation and vaccination, ear care
• Change-management skills and ability to support patients to change lifestyle
• Communication skills, both written and verbal
• Ability to communicate difficult messages to patients and families
• Negotiation and conflict management skills
• IT skills
• Team work
• Registered General Nurse

• Ability to identify determinants on health in the local area
