Job summary
An opportunity to become a Mental Health Practitioner in Malvern has arisen, which is a great opportunity to flex and develop your skills in working with people with a wide range of mental health-related needs in a creative way, supported by a close-knit, warm and friendly team.

Your role includes representing our service within a GP surgery, acting as a point of expert contact for GPs and other professions working within a defined locality. It also involves the exercise of considerable autonomy, though with support readily available and is perhaps unique in terms of the range and variety of people for whom you will be providing care, as well as being a role that involves learning skills for managing longer-term therapeutic relationships. Your working day will involve spending time both at our base with the team, as well as time out at GPs and in the community. Homeworking, in-keeping with the autonomy inherent to the role, is also available.

This is a full time role.

Main duties of the job
Travelling to your GP surgery in the morning to see two new patients for assessment. While there, dropping in on one of the GPs to talk about one of the assessments and run through another proposed referral to decide on the best mental health pathway for them.

Travel to base and write up the assessments and have the opportunity to talk through any concerns with fellow team members. You decide who you will be taking onto your caseload; no one allocates to you.

Attending professional meetings, including ward reviews via Teams or in person to review patients in your care

Attending MDT every Wednesday via Teams. Tend to work from home that day. Made some internal referrals to other team members and discussed some concerns about other patients you've been working with.

Meeting some of your longer-term patients and reviewing where you've got to with their care. Its good to work from a well-established relationship.

Knowledge
Essential

• Knowledge of principles of assessment and formulation for people with mental health problems
• Knowledge of principles of risk assessment and management
• Knowledge of Mental Health Act 1983 and Mental Capacity Act 2006 and other relevant legislation
• Knowledge of Personalised Care and Support Planning and principles of Personalised Care.

Desirable

• Knowledge of Care Act
• Experience of working with people in a forensic setting

Additional Criteria
Essential

• Will be required to complete mandatory training.
• Has a commitment to on-going learning, coupled with development of the role to meet patient and service need.
• To participate in regular locality duty system.

Experience
Essential

• Experience of working in a Mental Health setting
• Assessment of people presenting with mental health problems
• Assessment and management of clinical risk
• Working within a multi-disciplinary team

• Experience of Care Coordination/Key working.

Qualifications

• Relevant professional qualification relating to Health or Social Care e.g. in social work, nursing, OT with current professional registration
• RMN, BSc/DipSW, BSc OT
• Recent evidence of continuing professional development
• I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.

• Teaching / training qualification
• Psychological therapy qualification
