Job summary
We are seeking an experienced Advanced Practice Pharmacist to join our expanding multidisciplinary team to lead the development and delivery of our new in-house pharmacy service at Hollyns Health & Wellbeing. This is an exciting opportunity to shape an innovative, patient-centred service that supports high-quality, integrated primary care and improves health outcomes for our local population.

Working as an autonomous clinician within the MDT, you will provide expert pharmaceutical care across a broad range of clinical areas. You will undertake complex structured medication reviews, optimise prescribing, and promote the safe, and cost-effective use of medicines in line with national and local guidance.

As the clinical lead for the pharmacy service, lead quality improvement initiatives, clinical audit, and service evaluation, using data to inform practice and improve patient safety and outcomes. You will play a key role in developing new clinical pathways, implementing evidence-based practice, and supporting the strategic development of pharmacy services within the practice.

This role is ideal for an experienced independent prescriber with advanced clinical assessment skills, excellent leadership and communication abilities, and a passion for service development. The successful candidate will be committed to delivering high-quality, patient-centred care while driving continuous improvement and supporting the future growth of an innovative in-house pharmacy service.

Main duties of the job
  • Manage the daily work of our in house pharmacy service, working alongside a Clinical Pharmacist
  • Work closely with GPs, Clinicians and other practice staff to develop skills and support high quality, safe, cost effective prescribing
  • Demonstrate safe clinical decision-making and expert care for patients within general practice
  • Provide professional leadership, clinical supervision, and mentorship to the clinical pharmacist
  • Work collaboratively with the general practice team to meet patient needs

About us
Hollyns Health and Wellbeing is a proactive practice with two sites, who offer additional services to the practice population, such as warfarin, pain management, sexual health, Level 2 diabetic clinics and minor surgery. Teamwork is integral in ensuring the service is run as effectively and efficiently for the patients as possible. The successful candidate will be able to continue their service benefits if coming from the NHS in relation to the NHS Pension Scheme and annual leave entitlement. The Surgery has 12,300 patients approx. GPs, Nurses, Health Care Assistants, Pharmacists, Advanced Clinical Practitioners, Advanced Nurse Practitioners, Womens Newborn & Infant Practitioner and Administration staff. Chronic disease clinics for diabetes, asthma and COPD. Rated Good by CQC. Active GP and Nursing training and teaching practice. SystmOne clinical system. High QOF achievement. Part of a Primary Care Network with access to additional pharmacists, social prescribers, physiotherapy. Minor surgery, sexual health, diabetes, anticoagulation available for our patients.

Job responsibilities
  • Medication reviews, computer based or telephone calls for more complex patients, first source of information for patients about medication.
  • Medication and treatment reconciliation systems for patients transferred out of and back into the practice, assess unplanned admissions and systems to prevent further unplanned admissions.
  • Practice formularies maintain.
  • QOF ensure data connectivity and proactive organisation to deliver maximum QOF for the practice. Regular QOF audits. Improve risk stratification of pathways and improve service delivery.
  • Medicine management e.g. polypharmacy
  • Medicines optimisation, support joint goal setting with patients and appropriate treatment selection to support better medicines experience. Reducing waste and improving outcomes. Follow up identified patient groups targeting better concordance leading to improved outcomes.
  • Monitoring of long-term meds e.g. lithium, amiodarone, shared care medications and blood results.
  • Monitoring and managing repeat prescription wastage including patient adherence.
  • Optimising medication/titration e.g. HF
  • Lead and participate in Quality Improvement (QI) initiatives and clinical audits to enhance patient care, medication safety, and service delivery.
  • Plan, conduct, and evaluate quality improvement projects and audits, implementing evidence-based recommendations to support continuous service improvement.
  • Provide supervision, guidance, and mentorship to the clinical pharmacists, supporting their clinical development, competency, and professional growth.
  • Act as a clinical mentor and preceptor for clinical pharmacists, fostering a collaborative learning environment and promoting best practice in patient care.
  • Support the orientation, training, and ongoing development of junior pharmacists through regular coaching, feedback, and clinical supervision.
  • Work with clinical team members on long-term conditions (Diabetes, Asthma, COPD, Depression, Rheumatoid Arthritis, Hypertension, Arthritis, Pain, Heart Failure, Shared care drug monitoring).
  • Manage patients with more complex long-term conditions such as difficult hypertension, compliance with lipid-lowering therapy.
  • Assess and issue day to day acute medication requests as appropriate.
  • Address day to day medication queries.
  • Undertake repeat medication reviews and reauthorisation as appropriate.
  • Review and load repeat medication for new patient registrations.
  • Identify and support patients where concordance issues.
  • Liaise with community pharmacists to solve common day to day prescribing problems like preparation availability/alternatives etc.
  • Liaise with community pharmacists for the setting up, monitoring, and updating of all patients on NOMAD trays.
  • Liaise with community nurses and prepare MARS sheets as needed and support practice anticipatory prescribing process in liaison with community nurses and Community Pharmacy.
  • Liaising with care homes and community nurses re dressings.
  • Responsibility for lithium and antipsychotic drug monitoring and review of blood results.
  • Review the prescribing for patients with multi-morbidity and polypharmacy.
  • Report and follow up patient drug safety/adverse reactions.
  • Support patient self-care and medicines management and concordance through education.
  • Support staff education and development.
  • Support development of local services to improve workflow within the practice.
  • Work with clinical team members on long-term conditions (Diabetes, Asthma, COPD, Depression, Heart failure drug titration.
  • See and advise patients presenting with minor ailments presenting in surgery as appropriate to skills and competences.
  • Work with team to implement and maintain Practice formulary in longer term.
  • Development: support development of local services to improve workflow within the practice.
  • Service advancement; deliver specific disease management clinic through personal development and independent prescribing.

Person Specification - Experience
  • Have experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice.

  • To hold an MSc Advanced Clinical Practice with four-pillar advanced practice competencies and V300 Independent Prescribing
