Job summaryAn exciting and challenging opportunity has arisen to join our innovative and dynamic team within our expanding Frailty Virtual Ward. We are looking for an experienced and motivated senior occupational therapist or physiotherapist (Band 7) who are passionate about service development and providing high standard of patient centred care. You will be working closely within a Multi-Disciplinary Team of staff including Doctors, senior Frailty practitioner nurses, Occupational Therapists and Physiotherapists, all of whom are committed to maintaining independence, promoting self-care and assisting patients in their usual place of residence. At times this post may require you to work autonomously, when working across the Hackney borough into patients own homes and in an inpatient hospital setting. This post will be offered as a a full-time and permanent.
Main duties of the job- Provide a senior therapy assessment and treatment of complex, frail patients providing specialised high-quality care and support for patients in their own home to prevent unnecessary admissions and/or facilitate timely discharge from hospital/ other healthcare settings under direct or indirect supervision of senior medical colleagues.
- Participate in service development within Frailty Virtual Ward service set up, audits and outcome measures in relation to Frailty Virtual Ward Occupational Therapy or Physiotherapy practice and patient care.
- Work in a challenging socioeconomic environment involving patients who have highly complex social problems and those from culturally diverse backgrounds.
- Deputise for Band 8A Frailty Virtual Ward Clinical Lead in their absence taking responsibility for operationally managing allocated parts of the service.

Person SpecificationSkills- Ability to assess clients with complex physical disabilities
- Able to manage a clinical caseload and to prioritise as required.
- Able to problem solve, plan and manage time effectively
Experience- Post graduate experience in general physical field of OT or Physiotherapy.
- Experience of working in acute hospital setting
- Significant, recent experience working with elderly care and/or frail patients
Other- Demonstrates an understanding of Trust Values
Education / Qualifications- Degree / diploma in OT recognised by COT or Degree/diploma in Physiotherapy recognised by the CSP
