Job summary
An exciting opportunity has arisen for a motivated and compassionate Registered Mental Health Nurse to join the Hull Community Mental Health Team (CMHT). As a Band 5 Community Mental Health Nurse, you will play a key role in delivering high quality, recovery focused care to adults experiencing complex mental health needs across community settings.

Working as part of a supportive multidisciplinary team, you will undertake comprehensive mental health assessments, develop and deliver person-centred care plans, coordinate care under the Care Programme Approach (CPA), and provide evidence-based interventions that promote recovery, independence and social inclusion. You will work closely with service users, carers, primary care colleagues and partner agencies to ensure holistic and effective care is provided.

Main duties of the job

• Manage a defined caseload of adults with complex mental health needs, providing assessment, treatment, care coordination and ongoing review.
• Undertake comprehensive mental health assessments, formulate care plans and evaluate outcomes in partnership with service users and carers.
• Deliver evidence-based, recovery-focused interventions that promote independence, wellbeing and social inclusion.
• Assess, manage and respond appropriately to clinical risk, safeguarding concerns and crisis situations to ensure the safety of service users, carers and others

Job responsibilities

Qualifications and Knowledge
Essential

• Full understanding of relevant policy, legislation, drivers and their application to clinical and service area, e.g. Mental Capacity Act, Social Inclusion
• Full understanding/application of relevant clinical practice/standards/audit within identified clinical area
• Basic IT skills
• Part III or 13 of NMC live register
• Post registration/graduate diploma or equivalent experience in related work area, to demonstrate advanced level of practice
• Demonstrable experience of mentoring pre-registration students
• Prepared to undertake or hold a recognised non-medical prescribing qualification, if required

Desirable

• Evidence of project work that has impacted on practice and demonstrates a higher level of communication/planning/change that impacts within own area of work/responsibility
• Evidence of CPD or experiential learning at an advanced level e.g. Post graduate/Masters level
• Leadership training/qualification/experience

Experience
Essential

• Evidence of continuing professional development (CPD) relevant to the clinical area at specialist level of care
• Evidence of development of specialist practice nursing skills and able to demonstrate the impact of this on practice change/development
• Evidence of promoting/supporting active user/carer involvement/participation
• Act as Care Programme Approach (CPA) Co-ordinator

Desirable

• Demonstrable experience of working in the specific field where the post is held
• Development of specialist/advanced level of practice skills and able to demonstrate the impact of this on practice change/development
• Leadership/management experience which has had a positive impact and created change within the service delivery/practice
