Hywel Dda University Health Board are delighted to offer the opportunity for an experienced physiotherapist to join our Community Learning Disabilities Team (CLDT) in Carmarthenshire, West Wales.

You will work within a well-established multidisciplinary team supporting adults with learning disabilities to maximise their health, wellbeing, independence and quality of life. As one of four CLDT's within Hywel Dda UHB, the service offers strong professional networks, peer support and opportunities for collaborative development.

Alongside managing a specialist caseload, the successful candidate will undertake a Clinical Lead role in an agreed area of physiotherapy practice. We are particularly interested in applicants with an interest in spasticity management.

This role offers a highly specialist caseload, professional leadership support, regular supervision, continuing professional development opportunities, and involvement in service improvement, audit, education and innovation.

We are seeking a physiotherapist with significant post-registration experience, excellent communication skills and a passion for improving outcomes for people with learning disabilities. You will demonstrate strong clinical decision-making, evidence-based practice, caseload management and multidisciplinary working skills.

Main duties of the job
The post holder will work autonomously as a Clinical Specialist Physiotherapist, providing highly specialist assessment, treatment and management for adults with learning disabilities who present with complex physical and health needs.

They will manage a specialist caseload, using advanced clinical reasoning to develop, implement and evaluate evidence-based interventions. Working closely with individuals, families, carers, mainstream services and the multidisciplinary team, they will support people to maximise physical function, health outcomes, independence and quality of life.

The role includes making reasonable adjustments to ensure people with learning disabilities can access effective physiotherapy services, either by facilitating access to mainstream provision or providing specialist intervention where required.

The post holder will provide physiotherapy leadership within the Community Learning Disabilities Team and contribute to service development, audit, research and quality improvement initiatives. They will lead on a defined specialist area of Learning Disability Physiotherapy, supporting clinical practice, staff development and competency across the service.

Responsibilities also include contributing to policies, pathways, standards and training programmes, supervising and supporting physiotherapy staff, support workers and students, and deputising for the Professional Lead Learning Disabilities Physiotherapist when required.

Qualifications and Knowledge
Essential

• Degree or equivalent in Physiotherapy
• Professional registration with the HCPC.
• Evidence of postgraduate study at Master's level or equivalent demonstrable specialist knowledge acquired through extensive clinical experience, continuing professional development and relevant postgraduate training.

Desirable

• Injector qualification
• Clinical Educator Training Course.
• Evidence of leadership courses/ qualifications
• Completion of the Oxford Centre for Enablement Complex Posture Management course (4 days or Post Grad)
• Knowledge of Welsh NHS and government priorities for people with a Learning disability.
• Member of the chartered society of physiotherapy

Experience
Essential

• Substantial post-graduate experience in the core areas of LD Physiotherapy including: 24 hour postural management, management of mobility problems, rehabilitation, community level respiratory management, falls prevention and spasticity management.
• Evidence of significant post graduate experience working with adults or children with profound and multiple disabilities.
• Managing a complex caseload using an advanced clinical reasoning process.
• Experience of undertaking specialist assessment and management of adults with learning disabilities presenting with complex physical, communication and behavioural needs.
• Significant experience in multi-agency working within specialist area
• Experience contributing to multidisciplinary care planning and risk management processes.
• Significant experience working as an autonomous practitioner.
• Direct involvement with carers and families and demonstrating advanced communication skills both verbal and written during these interactions
• Evidence of supporting service compliance with clinical governance requirements
• Membership of clinical networks/ clinical practice groups
• Experience in assisting or leading service developments
• Experience in staff development
• Experience in presenting within clinical area

Desirable

• Experience of health promotion and preventative developments with service.
• Evidence of team management.
• Experience of service development across professional boundaries
• Experience delivering hydrotherapy and/or rebound therapy to individuals with complex disabilities.
• Experience in undertaking risk assessments in relation to people with complex needs.
