Job summary
Candidates are invited to apply for the post of Subspecialty training in Reproductive Medicine and Surgery, recognised by the Royal College of Obstetricians and Gynaecologists (RCOG) upon satisfactory completion of the training period, at Imperial College Healthcare NHS Trust. The post provides a fantastic opportunity to participate in a 24-month clinical training programme in Reproductive Medicine and Surgery in one of the busiest IVF Units in the UK, benefiting from the vast experience and knowledge of its consultants. The post is known for its long-established training programme, with the candidate given a breadth of exposure to an IVF unit offering the full range of treatments, reproductive surgery and endocrinology.

Main duties of the job
The applicant will need to be an NTN holder having completed a minimum of ST5 level in a recognised Obstetrics and Gynaecology training programme and will preferably hold sufficient research experience to apply to the RCOG for exemption from the research year of the Subspecialist training programme prior to the commencement of the post. Training will include active participation in the fertility service, the reproductive medicine, endometriosis, early pregnancy and recurrent miscarriage clinics, training in ultrasound as related to infertility and reproductive endocrinology, participation in general endocrine and paediatric endocrine clinics, participation in a programme of ovulation induction and in vitro fertilisation, including embryo cryopreservation, oocyte donation, intracytoplasmic sperm injection, as well as training in laboratory sciences related to reproduction. The post-holder will also participate in gynaecological surgery including minimally invasive reproductive surgery. The post holder will also be expected to take part in general Obstetrics and Gynaecology training, which is required to complete or maintain their advanced curriculum for CCT, including participation of the full on-call rota for the specialty and sub-specialty.

Job responsibilities
The post provides advanced subspecialty training in reproductive medicine and surgery, enabling the trainee to develop expertise in the diagnosis and management of infertility, reproductive endocrinology and fertility preservation.

The successful trainee will:
  • Provide specialist outpatient assessment and management of individuals and couples with infertility and reproductive endocrine disorders.
  • Participate in assisted conception services, including ovulation induction, intrauterine insemination (IUI), IVF/ICSI treatment planning, cycle monitoring, oocyte retrieval, embryo transfer and fertility preservation pathways, according to local competencies.
  • Develop advanced skills in reproductive endocrinology, including management of disorders such as premature ovarian insufficiency, amenorrhoea and menopause where relevant.
  • Undertake advanced diagnostic and operative gynaecological procedures relevant to reproductive medicine, including hysteroscopy and laparoscopic surgery for infertility, endometriosis, fibroids and tubal disease, under appropriate supervision.
  • Participate in multidisciplinary team (MDT) meetings, fertility governance, complex case discussions and patient counselling.
  • Contribute to the care of patients requiring fertility preservation before cancer treatment or other gonadotoxic therapies.
  • Participate in emergency and elective gynaecology services and maintain appropriate obstetric competencies in accordance with RCOG training requirements.
  • Contribute to teaching and supervision of junior medical staff, students and allied healthcare professionals.
  • Participate in clinical audit, quality improvement, governance activities and service development.
  • Undertake and contribute to research, innovation and publication within reproductive medicine.

Participate in the on-call rota as agreed within the training programme and local service requirements.

Person Specification
Education/ qualifications

Essential
  • MBBS or equivalent MRCOG

Desirable
  • Evidence of distinction at university or postgraduate examinations
  • Evidence of further advanced clinical training in relevant sub-specialties (e.g. ATSM, ASM, Diplomas)
  • Evidence of further training in research, such as Higher degree (MD/PhD) in relevant subspecialty, Advanced Professional Module (APM) in Clinical Research
  • Advanced Training Skills Module (ATSM) in Subfertility and Reproductive Health

Clinical expertise in Specialty/sub specialty

Essential
  • Completion of a minimum of 5 years of specialty training in Obstetrics and Gynaecology at time of appointment i.e. equivalent to ST6/7 with successful ARCPs
  • Experience of dealing with complex obstetrics and gynaecology emergencies
  • Comprehensive general training and experience of working in busy obstetrics and gynaecology unit at senior level of training with solid basic surgical skills

Desirable
  • Good progress or completion of ATSM or equivalent
  • Basic fertility experience

Teaching and training

Essential
  • Evidence of active participation in undergraduate and/or postgraduate teaching with feedback

Desirable
  • Completion of formal teaching course or qualification
  • Completion of educational supervision/mentoring course or experience as an educational supervisor

Communication

Essential
  • Capacity to communicate effectively with patients and staff both verbally and written
  • Capacity to take in others' perspectives and treat others with understanding and sensitivity
  • Capacity to discuss sensitive issues effectively with others and adapt language as appropriate to the situation
