Every moment counts. Every memory matters.
Come and join our nursing team as a Transition Nurse and play a vital role in supporting children and young people with life-limiting and life-threatening conditions, including those preparing to transition from children's services into adulthood.
Applicants for this role must be a Registered Children's Nurse (RSCN/RN Child), Registered Learning Disability Nurse (RNLD) or Registered Adult Nurse (RN Adult/RGN) and hold a Diploma or Degree qualification with active NMC registration. You will have experience of working with children and young people with complex needs and be passionate about delivering high-quality, family-centred care.
This is an exciting opportunity for an experienced and compassionate nurse who is committed to improving outcomes for children, young people and families. You will be based at our Dorset Hospice, supporting children, young people and families in hospice, home and community settings. Regular travel will be required to schools, colleges, hospitals, adult hospices and partner organisations.
As a Band 6 Nurse, you will hold a caseload of children and young people and act as a Named Nurse, ensuring the ongoing assessment, planning, implementation and evaluation of care. You will develop strong therapeutic relationships with families, providing continuity, coordination and advocacy whilst ensuring that care remains centred around the individual needs, wishes and aspirations of each child and young person.
A key aspect of this role is supporting young people through transition into adulthood. You will hold a dedicated caseload of young people in transition, supporting them and their families from approximately age 14 onwards. Acting as a key point of contact throughout the transition journey, you will coordinate person-centred transition planning, helping young people prepare for adult services whilst ensuring their goals, aspirations and changing needs remain at the heart of decision-making.
Working closely with colleagues across Julia's House and with external health, social care, education and adult service providers, you will help develop and embed effective transition pathways that improve experiences and outcomes for young people moving into adult services. You will provide specialist advice and support relating to transition planning, advocacy, the Mental Capacity Act, Best Interests Decision Making, Deprivation of Liberty Safeguards and Liberty Protection Safeguards, helping families navigate what can often be complex systems and services.
Alongside your specialist transition responsibilities, you will be an active member of the clinical nursing team and will be rostered to provide hands-on respite, palliative and end-of-life care. Direct care delivery forms a significant part of this role and includes supporting children and young people with complex health needs in hospice, home and community settings. You will work collaboratively with Carers, Nurses and the wider multidisciplinary team to ensure care remains safe, responsive and centred around the needs of children, young people and families.
You will participate fully in the clinical rota, working a combination of day, evening, weekend and night shifts in line with service requirements and the needs of the families we support. You will maintain and develop your clinical skills, contribute to clinical decision-making and support the delivery of high-quality nursing care across our Dorset and Wiltshire services.
The role also involves developing strong partnerships with adult health services, hospices, social care teams, education providers, commissioners and community organisations. You will contribute to education and training initiatives, support colleagues to develop confidence in transition practice and help evaluate and improve services through audit, feedback and quality improvement activity.
If you are passionate about helping children and young people achieve the best possible quality of life, including supporting them to navigate the journey into adulthood, and are looking to develop your career within children's palliative care, we will support you through a wide range of opportunities for clinical and professional development.
This is a permanent, full-time position. Full-time hours at Julia's House are 35 hours per week. A full driving licence and access to a vehicle insured for business use are essential, and all appointments are subject to a satisfactory Enhanced DBS check.
At the discretion of the charity, relocation assistance may be considered.
Flexibility will be required to cover days, evenings, weekends and nights in line with service requirements. There are no set working hours for contracted staff, as our care is delivered around the needs of the children, young people and families we support. Rotas are prepared at least six weeks in advance.
