We are seeking a Band 6 Wound Clinic Team Leader to join our Tissue Viability and Wound Therapy Service, providing high-quality, evidence-based wound care to patients attending Medway Community Healthcare wound clinics.

The successful candidate will work as part of a specialised multidisciplinary team, assessing patient needs, developing and evaluating personalised care plans, and delivering appropriate wound treatment, prescribing, advice and support to promote healing and improve outcomes.

You will support the effective day-to-day management of wound clinics, assist the Senior Wound Therapist, and lead workload allocation during short periods of absence. The role includes providing leadership, supervision and development support to Healthcare Assistants, students and visitors, ensuring safe, compassionate and high-quality care aligned with current evidence and best practice.

Main duties of the job

• Assess, treat and monitor patients attending wound clinics, delivering evidence-based wound care and evaluating treatment outcomes.
• Conduct comprehensive wound assessments and develop individualised care plans in collaboration with patients and the multidisciplinary team.
• Provide education, advice and support to patients, families and carers to promote wound healing, self-management and health improvement.
• Communicate effectively with GPs, community services, care homes, hospitals and other healthcare professionals to ensure coordinated patient care.
• Organise and prepare clinic sessions, prioritising workloads and managing appointments to meet patient needs efficiently

Qualifications, training and experience
Essential

• You will have a Level 6 Degree or equivalent qualification, or equivalent knowledge acquired through experience plus a diploma in nursing.
• You'll need to have experience of managing complex caseloads and supporting/ mentoring others.
• You will be a registered health care professional with the Nursing and Midwifery Council

Knowledge and skill
Essential

• You will need to be able to be able to use IT systems and databases.
• You will have knowledge of up to date clinical practice by using up- to- date skills and adhering to evidencebased procedures and clinical effectiveness frameworks. You will fully understand the role and remit of a health care worker.

Desirable

• Having an awareness of developments within the NHS would be an advantage as well as an understanding of National Service Frameworks and NICE guidance.

Working environment
Essential

• A full, valid UK driving licence or a full driving licence issued by an EU country (not exchanged from a non-EU country) and the ability to travel independently are essential for this role.
