Job summaryYour day-to-day work will focus on the autonomous management of patients with minor illnesses from a pre-determined and agreed list of presentations. You must constantly strive to make the care of your patients your primary concern whilst being a patient advocate. The role at times will be to act as a senior presence to other members of the team with the potential to develop and will include patient contacts within the practice and home visits.

Job descriptionMain DutiesClinical- Utilise assessment and clinical reasoning skills to triage, examine, provide treatment, and make relevant referrals for patients who present with an urgent healthcare need within defined and agreed parameters dependent of clinician competence.
- Use appropriate assessment tools and clinical guidelines to assist with clinical decision making (NEWS, POPS, CRB-65).
- Ability to assess capacity and understand that competent patients are within their right to make a bad decision.
- Follow practice policies and protocols whilst undertaking the practitioner role.
- Refer patients to an Advanced Practitioner or General Practitioner when required.
- Prescribe and review medications within competence in line with the Royal Pharmaceutical Societys Prescribing Competency Framework.
- Within competence, perform, request, and interpret a variety of tests relevant to the role.
- Assist and support nonclinical staff who might have queries and questions regarding patient needs.
Leadership- Role model and act as a senior practitioner to support colleagues supporting staff of all levels.
- Demonstrate an understanding and participate in clinical governance practices.
- Support and mentor students and colleagues of all disciplines when required.
- Provide teaching sessions and supervision to junior practitioners.
- Participate in clinical audits when requested by line manager.
Learning- Keep an up-to-date professional portfolio and log of evidence in preparation for CQC inspections.
- Actively participate in Continual Professional Development (CPD).
- Participate and attend clinical meetings where indicated.
- Keep up to date with mandatory training relevant to the role.

- Independent Prescribing Qualification.
- Physical Assessment and Clinical Reasoning Qualification.
- Experience managing patients with complex needs from diverse background.
Desirable- Paediatric assessment training/module
- Working towards or have achieved one of the following relevant qualifications: Level 6 Graduate Certificate/Diploma, Level 7 Post Graduate Certificate/Diploma, Level 7 Masters degree
ExperienceEssential- Worked within a GP Surgery.
- Experienced managing patients with complex needs from diverse backgrounds.
- Understanding of safeguarding adults and children.
- Understanding and knowledge of healthcare provision in GP surgeries, QOF and enhanced service.
Desirable- Experience of working in primary care
