Job summary
You will work closely with the inpatient wards in the Bay and HBTT to support patients be safely discharged to their own homes or appropriate accommodation. You will work to support patients be discharged from hospital and engage with HBTT. You will be focused on developing community links preventing hospital admission and will ensure the smooth transition for service users between specialist and mainstream services and other teams.

Main duties of the job
You will be a Registered Mental Health Nurse, Approved Social Worker, Occupational Therapist or Dip SW with evidence of professional development. You should have some post qualifying experience and should have experience of working with people who have a severe mental illness within an acute environment. Some experience of working within a community setting may also be beneficial.

We can offer you supervision, training and support. You will be part of a team and be included in the developments moving the service forward.

Qualifications

• Registered Mental Health Practitioner

Experience

• Able to understand the difference between crisis planning and contingency planning and able to formulate explicit planning in these areas
• Demonstrate the ability to work sensitively in a non judgemental way with service users who are in crisis
• Assertiveness skills in relation to bed management
• Complete risk assessments which reflect the risk a service user may pose.

Knowledge

• Able to understand the difference between crisis planning and contingency planning and able to formulate explicit planning in these areas
• Knowledge, understanding and experience of the care pathways for psychosis and non psychosis and relevant interventions
• Understanding of how physical illness and substance misuse can affect the mental health of individuals
• Knowledge of Safeguarding procedures, Criminal Justice procedures, Mental Health legislation, guidance and processes,
