Description

Job summary
An exciting opportunity has arisen for a Registered Mental Health Nurse, Social Worker or Occupational Therapist to joint the Pennine West Home Based Treatment Team. We are looking for a highly motivated, innovative and a dedicated practitioner to provide care and treatment for those service users who require support from the Home Based Treatment Team.

This dynamic and driven multi-disciplinary team offer an alternative to psychiatric hospital admission, supporting service users in acute mental health crisis in their homes, as well as supporting loved ones and carers during this period.

The team are passionate about providing person centred care, tailoring care plans to suit the needs of all individuals involved during the care spell and providing a range of psychosocial interventions during their admission.

The team are required to thoroughly risk assess situations and have access to a supportive leadership team to support such complex cases.

Staff wellbeing and development is paramount and well supported in this team.

The post holder will be supported with their professional development as part of their appraisal process, and regular 1:1 supervision is provided.

You will have experience of working with service users who present with complex mental health needs.

Main duties of the job
The post holder will:
  • Undertake collaborative care planning with service users and their carers.
  • Undertake risk assessment and record risks in line with the trust policy
  • Undertake shift co-ordination when allocated to do so.
  • Participate in Multi-disciplinary Team (MDT) and agree plans with patients in relation to their care.
  • Maintain their knowledge of evidence based home treatment interventions and deliver these interventions in line with service use care.
  • Undertake collaborative care planning with services users and their carers.
  • Undertake gatekeeping assessments to ensure service users are treated in the least restrictive environment.
  • Undertake allocation of shift co-ordination.

You should be able to demonstrate evidence of care that facilitates a positive culture of patient engagement, a commitment to enhancing the service user and carer experience and of on-going professional development.

You will have experience of implementing effective, evidence based care management / assessment skills and display excellent communication skills.

The post holder will be able to work independently as well as being committed to a multi-disciplinary team Approach (MDT) both of which are essential requirements for the post.

Education / Qualifications
Essential
  • Registered Mental Health Practitioner

Desirable
  • Practice teacher qualification/student supervisor

Knowledge
Essential
  • Able to understand the difference between crisis planning and contingency planning and able to formulate explicit planning in these areas
  • Knowledge, understanding and experience of the care pathways for psychosis and non-psychosis and relevant interventions
  • Effective communication with service users, carers and members of the multidisciplinary teams. Strong customer service and negotiation skills

Experience
Essential
  • Motivational skills
  • Assertiveness skills in relation to bed management
  • Demonstrate the ability to work sensitively in a non-judgemental way with service users who are in crisis

Personal
Essential
  • Ability to meet the travel requirements of the role
  • Ability to work a 24 hour shift pattern
