Job summary
We are seeking a compassionate, enthusiastic Advanced Nurse Practitioner (ANP) to join our friendly and supportive team at the Langton Grange Medical Group. The candidate will have previous experience in general practice.

This role is suitable for both experienced ACPs and those who have recently qualified.

This role will involve working at all our three sites within Langton grange Medical Group: Lichfield, Whittington and Burntwood.

The successful candidate will deliver high-quality, patient-centred care, working autonomously to assess, diagnose, treat, and manage a broad range of acute, minor, and long-term conditions. You will manage patients presenting with undifferentiated, undiagnosed issues carrying out full clinical assessments, initiating treatment plans, prescribing, and evaluating care in line with national guidelines.

You will also contribute to the ongoing development and innovation of the practice, playing an active role in service improvement and patient care delivery.

Main duties of the job
  • Provide expert clinical assessment and diagnosis for patients presenting with undifferentiated, undiagnosed, and complex health problems.
  • Initiate, evaluate, and review treatment plans, including prescribing medications and therapies within your scope of practice.
  • Manage a range of acute, minor, and long-term conditions independently, referring to or liaising with GPs or secondary care where appropriate.
  • Undertake comprehensive physical assessments, using advanced clinical skills and decision-making.
  • Perform and interpret relevant investigations, such as blood tests, ECGs, and other diagnostic procedures.
  • Promote preventative healthcare and offer patient education, self-care advice, and health promotion.
  • Work collaboratively with the multidisciplinary team to ensure high standards of care, continuity, and efficiency.
  • Contribute to clinical governance, audits, significant event analysis, and the development of local policies and protocols.
  • Participate in service development and innovation, supporting quality improvement initiatives across the practice.
  • Maintain accurate and timely patient records in accordance with legal, ethical, and professional standards.
  • Support junior staff and students through mentoring, supervision, and education where applicable

We are a well-established General Practice based in Lichfield, serving a patient population of approximately 13,500 across three sites: Lichfield, Whittington, and Burntwood. Our multidisciplinary team includes experienced GPs, Advanced Nurse Practitioners, Pharmacists, Practice nurses and HCAs working collaboratively to deliver high-quality, patient-centred care to our community.

Job responsibilities
  • Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice
  • Provide advanced clinical assessment, diagnosis, treatment, and management of patients with both acute and chronic conditions.
  • Request pathology services where required
  • Process and interpret pathology and other test results as required
  • Conduct independent prescribing in line with professional and legal frameworks.
  • Work autonomously and collaboratively with GPs, nurses, and the wider multidisciplinary team to deliver holistic, evidence-based care.
  • Manage a wide range of minor illness and long-term conditions, including triage, treatment planning, and referral when necessary.
  • Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health
  • Take a lead role in chronic disease management and contribute to achieving Quality and Outcomes Framework (QOF) targets through proactive patient reviews and recall systems.
  • Ensure accurate and effective clinical documentation using SNOMED CT coding to support data quality, clinical audits, and reporting.
  • Promote health education, disease prevention, and patient self-care in line with public health strategies.
  • Support and contribute to clinical governance, including clinical audits, significant event reviews, and evidence-based practice improvements.
  • Support the clinical team with all safeguarding matters in accordance with local and national policies
  • Participate in the development of clinical protocols and contribute to service innovation and delivery improvements.
  • Provide mentorship and supervision to junior clinicians, students, and other practice staff as appropriate
  • Attend clinical, MDT and practice meetings
  • Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner
