Job summary
Laurel House Surgery is looking to recruit a motivated and enthusiastic Pharmacy Technician to join our Pharmacy Team.

Working as part of our Pharmacy Team and alongside our GPs, Nurses and wider multidisciplinary team, you will support the safe and effective management of medicines within the practice. You will assist with medicines optimisation, repeat prescribing, medication reconciliation, prescribing safety and medicines management activities, helping to ensure patients receive high-quality, patient-centred care.

The successful candidate will work collaboratively with colleagues across the practice to improve prescribing processes, enhance patient safety and contribute to the efficient delivery of pharmacy services within Primary Care.

Main duties of the job
The successful candidate will:

• Support the Pharmacy Team with medicines optimisation activities.
• Process repeat prescription requests in accordance with practice policies and national guidance.
• Undertake medication reconciliation following hospital discharge.
• Update and maintain accurate patient medication records using the practice clinical system.
• Identify patients requiring medication reviews and refer them appropriately within the Pharmacy Team.
• Respond to routine medication queries from patients, pharmacies and healthcare professionals within the scope of the Pharmacy Technician role.
• Assist with prescribing safety audits and quality improvement projects.
• Support patient recall programmes for medication monitoring.
• Liaise with community pharmacies, hospitals, care homes and other healthcare providers regarding medication-related matters.
• Promote safe, effective and cost-efficient prescribing in line with local and national guidance.
• Work collaboratively with the multidisciplinary team to improve patient outcomes.
• Maintain accurate clinical records and comply with confidentiality, GDPR and practice policies.
• Participate in mandatory training and continuing professional development.
• Undertake medication reviews within your competency
• Action MHRA safety alerts
• Monitor shared care agreements

Person Specification
Qualifications
Essential

• Registered Pharmacy Technician with the General Pharmaceutical Council (GPhC)
• Recognised Pharmacy Technician qualification (Level 3 or equivalent)
• Experience working as a Pharmacy Technician
• Knowledge of medicines optimisation and repeat prescribing
• Understanding of medication reconciliation processes
• Excellent communication and interpersonal skills
• Good organisational skills and ability to prioritise workload
• Excellent attention to detail
• Good IT skills
• Ability to work independently and as part of a multidisciplinary team
• Commitment to patient confidentiality and professional standards
• Willingness to undertake continuing professional development

Desirable

• Experience within General Practice or Primary Care
• Experience using EMIS Web or other GP clinical systems
• Knowledge of NHS prescribing guidance and medicines management
