Job summaryLeeds and York Partnership NHS Foundation Trust is the lead provider for perinatal services for the Yorkshire and Humber provider collaborative. There will be a further bed expansion to the existing and only mother and baby unit in Yorkshire and Humber, which will allow for inpatient care to be provided for mothers and their infants closer to their home. The mother and baby unit provides acute inpatient care for women and their infant (from 32 weeks of pregnancy up to 12 months postnatal) who are suffering from a moderate to severe mental illness.
Main duties of the jobAn exciting opportunity has arisen for an enthusiastic and motivated individual(s) to join our innovative, dynamic and forward thinking perinatal mental health referrals management service. The Referrals Management Service was established to coordinate referrals from across the region for the Yorkshire and Humber Mother and Baby Unit. This is a new and developing part of the existing perinatal mental health community provision, providing opportunities for the successful candidate to contribute to the development of the service. The post holder will be required to work across the perinatal community service and mother and baby unit, whilst liaising with acute mental health wards, community mental health services, health professionals and social care across the region. The post holder will attend CPA meetings for service users placed out of area and in acute mental health wards. Experience of triaging referrals and liaising with professionals would be advantageous for the role.
Some of our families have very complex social circumstances and this needs sensitive handling alongside the management of the mental illness and the ongoing safeguarding of mother and infant. The post holder will lead on partners assessments and the initiation of early help plans to support wider family.

Job responsibilitiesThe successful candidate will hold a qualification as a Registered Mental Health Nurse, Occupational Therapist or Social Worker with experience of working in mental health care. They should have experience of working autonomously and as part of a multi-disciplinary team with an ability to think strategically and demonstrate excellent time management skills. This is an exciting opportunity to join a team that is committed to working in a multi agency recovery focussed way with mums, families and wider social networks across all areas of our service. As a specialist service, we prioritise teaching for partner agencies and the successful candidate will have the opportunity to contribute to this.
- To work as part of a team to manage, triage, and assess referrals that are made to the regional mother and baby unit and at times the community perinatal service.
- To participate in multi-disciplinary and multi-agency working, being aware of colleagues' caseloads and providing support to others as necessary and contribute to the on-going development of the Leeds Perinatal Service.
- To maintain comprehensive and up to date written records of assessments, CPA plans, evaluations and on-going process notes in line with Trust Policy and to share these with the MDT and external agencies as appropriate.
- To ensure effective communication and collaboration with others through attendance at, and chairing of CPA meetings, team meetings, ward rounds, clinical meetings and meeting held by partner agencies aimed at addressing the welfare of the mother, child and wider family.
- To ensure the service user and their families are central to and fully involved in the plan of care and have copies of care plans.
- Imparting sensitive and complex information to service users and their families.
- To be aware of and adhere to all professional and Trust Policies including the reporting of incidents and accidents, complaints procedures and the management of confidential information.
- To work collaboratively and communicate effectively across the primary care secondary care and children's social care agencies.
- To support the Mother and Baby Unit and Community Services when planning discharge.
- To offer partners assessments and support whilst the mother is admitted to the mother and baby unit.
- Liaising with local services for support and planning upon discharge from the mother and baby unit.
