Job summary
To work as a member of FOLS in providing Occupational Therapy (OT) in a variety of community settings, including the service user's own home, and offer in-reach support to Clifton House Forensic wards pre-discharge to the community.

To establish and work within a clear OT pathway; facilitating screening assessment, needs analysis/prioritisation, treatment planning and facilitating interventions.

To be responsible for the assessment and planning of occupational and vocational interventions across the FOLS caseload; assessment of care needs, development, implementation and evaluation of interventions/care.

To work compassionately and collaboratively with service users to achieve identified goals, promoting social inclusion, independence and well-being.

To promote OT values and interventions in the MDT and with service users.

To take a professional lead for the overall provision and development of high-quality OT provision within FOLS.

To promote a positive image of OT and the wider Trust.

To establish and maintain good working relationships with other community teams/agencies.

To carry continuing responsibility for the setting and monitoring of standards of care in line with Clinical Governance requirements.

To actively participate in clinical supervision.

Main duties of the job
Communication & Relationships
Work as part of a multidisciplinary team (MDT), communicating effectively with service users, carers, colleagues, and partner agencies.

Attend and contribute to team, clinical and MDT meetings.

Use effective verbal and non-verbal communication to engage service users with varying cognitive and communication needs.

Clinical Practice
Manage workload and prioritise competing demands effectively.

To participate in the initial assessment of service users referred to the team

Decision Making & Team working
Use professional judgement and clinical reasoning to inform assessment, intervention and treatment decisions.

Work autonomously within professional competence and agreed risk management plans.

Physical & Practical Requirements
Use electronic patient record systems and other IT systems effectively.

Travel between Trust sites, service users' homes and community settings.

Service Development, Quality & Research
Contribute and commit to service development, evaluation, audit and quality improvement activities.

Support the development of OT offer and pathway within the team.

Professional Responsibilities
Maintain HCPC registration and comply with the Code of Professional Conduct.

Participate in supervision, appraisal and continuing professional development (CPD).

Person Specification
Qualifications

• Relevant core professional qualification that is essential for the post - BSc/Dip/MSc OT. HCPC registration.
• Evidence of post qualification training or on-going development.
• Evidence of training to support students in practice (SLIP/APPLE) or willingness to complete.

Skills

• Ability to work as a member of the MDT and has a positive approach to team working.
• Ability to consult, liaise and negotiate with service users, carers, family and other professionals.

Experience

• Demonstrates a high level of clinical skill, knowledge and experience.
• Experience of working with mental health.
• Experience of working within a multi-disciplinary team (MDT).

Knowledge

• Knowledge of relevant mental health legislation and policy.
• Good understanding of other services required to input into service users plans of care.
