Job summaryThe opportunity has arisen for an exciting Memory Assessment Practitioner role, working across the county to actively participate in the Memory Assessment pathway.
This is a 12-month fixed-term or secondment opportunity and is part of a dynamic review of how we deliver our memory services to broaden access and ensure the delivery of specialist memory assessments in a timely manner. It is essential to enable the timely diagnosis of dementia, supporting access to early treatment and support, informed decision-making, and optimal management and life outcomes for those in need.
The successful candidate will be required to competently assess patients, as well as plan and deliver assessment outcomes. This includes delivering dementia diagnoses, where appropriate, and discussing medication options where applicable.
The service is currently county-wide; however, we particularly encourage applications from candidates based in the south of the county. Frequent travel to bases across the county will be required to support service needs. This will be balanced with home-based working, as the post is a hybrid working role.
Applicants must have the right to work in the UK - sponsorship is not available. Please provide details of the visa type and expiry date in your application. Failure to provide the required information will result in your application being rejected.
Main duties of the jobThe post currently involves undertaking remote (via video call) or clinic-based appointments in line with the Memory Pathway and Protocols.
You will be responsible for making the initial contact with patients and carers to support their understanding of the memory pathway, and for undertaking detailed history-taking using a holistic memory assessment framework. This includes the initial collection of all clinically relevant service user information and clinical history, while involving carers using agreed service formats.
There is a requirement to accurately and contemporaneously record and document clinical discussions and assessment details on the RiO clinical system.
Your comprehensive assessment and detailed history-taking will be presented to the multidisciplinary team. You will then communicate the findings, including any diagnosis, to the person assessed and, where appropriate, their loved ones. You will also provide monitoring of any recommended treatment, where suitable, in line with the Memory Assessment Pathway.
