Job summaryAn opportunity is available for an experienced and enthusiastic Mental Health Practitioner to provide specialist mental health care within the Integrated Placed Based Team in the Sleaford area.

Within this role, you would work as a valued member across primary and secondary care and with the neighbourhood teams to locally support the mental health transformation and improve the lives of people who access healthcare services.
Main duties of the jobThis role is unique and gives excellent opportunity for joined up working across services. This not only ensures appropriate and needs led outcomes for the patients, but also offers the practitioner opportunities for creative approaches to working and ensuring the patient remains central in recovery planning.
The successful candidate will be an experienced clinician with proven skills in working with people who have a range of highly complex mental health needs, demonstrating practice that is evidence-based and innovative. They will demonstrate adaptability, excellent communication skills and self-motivation. In addition to undertaking mental health assessments, you will take on a caseload of highly complex service users to provide therapeutic interventions and coordinate their care.
A comprehensive trust induction will be offered and opportunities for CPD. Management and Clinical supervision are provided regularly.

Person SpecificationExperienceEssential- Post- Registration experience of working with adults with mental health needs
- Sound knowledge of the national agenda for mental health
- Sound knowledge of clinical/ risk assessment and understanding of confidentiality
- Sound knowledge of community care issues, local health and social structures, functions and boundaries
- Sound knowledge of the Mental Health Act.
Desirable- Experience of leading training sessions
- Experience of neighbourhood working
Skills and CompetencesEssential- Highly developed clinical reasoning skills
- Prioritisation and resource allocation, planning and organisational skills.
- Able to communicate complex and at times highly sensitive information to a variety of stake holders.

QualificationsEssential- Degree in mental health and social care or equivalent:
- 1st Level Registration (NMC,HCPC)
- Evidence of specialist training at diploma level or equivalent
- Evidence of specialised continued professional training (degree level) in clinical practice.
- Evidence of continuous professional development
