LPFT and South Lincolnshire Rural Primary Care Network are seeking to recruit a Senior Mental Health Practitioner (Registered Mental Health Nurse, Occupational Therapist or Social Worker) to support patients registered with The Deepings Practice and Bourne Galletly Practice.

The successful candidate will deliver recovery-focused interventions that promote independence, resilience and positive outcomes for patients and carers. Working across primary and secondary care, the post holder will provide specialist mental health assessment, advice and interventions, supporting people with serious mental illness.

The role offers opportunities for creative and innovative ways of working, enabling practitioners to work autonomously while keeping the individual at the centre of assessment, care planning and intervention. The post holder will manage a caseload of complex cases, work collaboratively with GP practices, neighbourhood teams and secondary mental health services, and contribute to MDT meetings to ensure coordinated, high-quality care.

Main duties of the job
The successful candidate will be an experienced clinician with proven expertise in supporting individuals with highly complex mental health needs. You will demonstrate evidence-based practice, strong clinical decision-making, and a commitment to delivering innovative, person-centred care.

Key responsibilities will include:

• Working autonomously within designated GP practices, providing specialist mental health assessment, advice, consultation, and interventions.
• Delivering specialist clinical support to people with serious mental illness, promoting both mental and physical wellbeing.
• Undertaking holistic, person-centred assessments and planning, implementing, monitoring, and reviewing therapeutic interventions with individuals and carers.
• Working closely with secondary mental health services and partner agencies to develop personalised care plans focused on what matters most to the individual.
• Providing mentorship, guidance, and training, including Primary Care and Neighbourhood Multidisciplinary Team (MDT).
• Attending and contributing to neighbourhood and integrated team meetings, working collaboratively with the Neighbourhood Lead and wider partners.
• Facilitating and contributing to case discussions, case conferences, and MDT meetings to ensure coordinated, high-quality care and positive outcomes.
• Supporting a joined-up approach to care, ensuring individuals can access appropriate services, community resources, and interventions to meet their needs.

Qualifications
Essential

• Degree in mental health and social care or equivalent
• 1st Level Registration (NMC,HCPC)
• Evidence of specialised continued professional training (degree level) in clinical practice
• Evidence of advanced clinical practice at post graduate level to masters or equivalent
• Evidence of continuous professional development
• Non-medical prescriber (or ability to complete within first 12 months in post)

Desirable

• Management or leadership qualification

Experience
Essential

• Post-Registration experience of working with adults with mental health needs
• Experience of working autonomously within a senior position
• Experience of managing staff and good leadership skills
• Experience of working across organisations
• Sound knowledge of the national agenda for mental health
• Sound knowledge of clinical/ risk assessment and understanding of confidentiality
• Sound knowledge of community care issues, local health and social structures, functions and boundaries
• Sound knowledge of the Mental Health Act

• Experience of leading service change/development/transformation
• Experience of leading training sessions
• Experience of neighbourhood working

Skills

• Highly developed clinical reasoning skills.
