Description

Job summary
This role is an exciting opportunity to work directly with individuals in their community to teach and support individuals around mental ill health in Primary Care. This role is essential in supporting individuals to manage their mental health at the first stage of ill health and prevent further deterioration in their needs. The role is flexible and we promote hybrid working opportunities.

Do you believe in change to our current systems, introducing new ideas and integrated working with the holistic care of the service user at its heart? Do you want to work within a GP surgery as the Mental Health subject matter expert? We want you to be in a role where you can guide, support, listen and educate on mental health for both our service users and our wider NHS colleagues to ensure individuals get the right, most appropriate mental health care dependant on their needs.

Main duties of the job
Work within Meridian PCN to offer consultations within surgeries to meet the mental health needs of local communities at primary care stage. The role is offering intervention, mental health support and advice to our population and linking in with those in secondary care if and when appropriate to ensure the needs of individuals are being met. You would work closely with secondary care colleagues to ensure we are working within the principles of NHS Transformation 10 year plan.

About us
Lincolnshire Partnership NHS Foundation Trust provides mental health services and a number of learning disability, autism and social care services in the county of Lincolnshire. Employing around 2,900 staff, and serving a population of over 768,400, our people lie at the heart of everything we do.

You could be part of a Trust rated by staff as one of the best mental health and learning disability trusts in England. We firmly believe the key to high quality care is a contented workforce. This is reflected in our Care Quality Commission rating of 'outstanding' for well-led and 'good' overall. In the most recent National NHS Staff Survey, our staff rated us as the number one trust nationally for staff morale and one of the top scoring NHS Trusts in the Midlands for being compassionate and inclusive. We're really proud of this!

We offer options for flexible working and provide a wide range of training and promotion opportunities in all professions. We support and celebrate diversity, have active staff networks groups and are always looking at what more we can do to support our staff.
