Description

Job summary
The post holder will be expected to provide clinical knowledge, demonstrating a sound understanding of the issues related to the identification, assessment, treatment, and management of patients at risk of developing cardiovascular disease (CVD). The post holder will be required to travel across Cheshire and Merseyside to deliver the service to patients in their local area. The postholder will support the development, and lead on the day to day running of the CVD prevention service. They will manage the caseload referred to the service, monitor and improve standards of care through, clinical audit, evidence-based practice, supporting professional colleagues and the provision of skilled professional leadership. They will actively contribute to the clinical governance processes within the service.

The successful candidates will have continuing support and mentorship and there will be a strong emphasis on personal development, mentorship, education and support.

Main duties of the job
Clinical practice
  • Responsibility for assessment, planning, and developing an individualized management plan of care for patients within the CVD Prevention service, recommending these to patients GP
  • Clinically assess patients referred with incidental findings or multiple risk factors of CVD in a community clinic setting.
  • Refer patients for further investigation, with the support of the MDT and manage the patient though the service.
  • Educate patients and their families about their condition.
  • Liaise with the multi-disciplinary team to discuss patient care pathways
  • Liaise with the adult Familial Hypercholesterolemia / Diabetes and other specialist teams across the ICB region to provide a pathway for raising awareness
  • Work with primary care staff, regional consultants, physicians, nurse specialists and other related professionals to develop high levels of care for patients at risk of CVD
  • Actively engage with patients to obtain feedback from patient and family experience
  • Undertake opportunistic diagnostic testing for the public and certain sectors using point of care testing. This will involve discussing information to inform and counsel individuals on the significance of such investigations both for the individual and their family members and relay information to the relevant healthcare professions
  • Provide advice and written information as required in response to clinic appointments.

Job responsibilities
An exciting opportunity has arisen for a highly motivated cardiovascular disease healthcare practitioner to join our Strategic Partnerships team, working with the Cardiovascular Disease Prevention (CVD-P) Service.

The successful candidate will be expected to provide comprehensive assessment for patients identified with increased risk of developing cardiovascular disease. They will be responsible for the management and care of patients at risk of CVD including making observations of the patients physical presentation, such as blood pressure, heart rate, body mass index, point of care testing, psychological and emotional state during any reviews and report back to the clinical team. They will ensure that a high standard of patient care is provided, and that the principles of evidence based practice and best value are integral to practice and the core business objectives, in improving services for users in line with the patients vision.

You will need a positive outlook, with a desire to deliver quality service in a fast-paced environment. We are looking for someone who has a flexible approach with the ability to work on their own initiative and as part of a team, excellent organisational and time management skills and the ability to work under pressure and to tight deadlines. There is a requirements to travel around Merseyside and Cheshire area to provide clinics in the community.

The successful candidate will be expected to communicate with a wide range of people, from patients and their families, other health care professionals and external agencies.

Support from the Consultant and the Clinical Business Manager for will be given to develop your clinical skills.

Qualifications & Education
Essential
  • Professional qualification
  • Clinical / physical examination or clinical history taking qualification (or willing to complete the course in the first 2 months in post)
  • At least 2 experience within CVD speciality
  • Degree level education or working towards or equivalent
  • Evidence of commitment to continued professional development

Desirable
  • Teaching and assessing qualification or equivalent
  • On Professional register or working towards

Knowledge & Experience
Essential
  • Knowledge of development and treatments for cardiovascular risk factors
  • Evidence of research and audit skills
  • Experience of providing advice and training to GP's and other healthcare professionals
  • Ability to work within a multi-disciplinary team

Desirable
  • Understanding of the Data Protection Act
  • Counselling/motivational interviewing skills

  • Familiarity with databases and word processing, use of VDU's and digital technologies
