Advanced Nurse Practitioner - Inflammatory Bowel Disease

Job summary
The post holder will be an autonomous practitioner who will be expected to take an active role in the respond, stabilise and make safe element of acute care pathways by delivery of advanced assessment, and the management and treatment of patients over 18 years of age patients with inflammatory bowel disease (IBD) referred to IBD Hot Clinic.

Main duties of the job
The Advanced Practitioner role, within the IBD service, will provide comprehensive healthcare sensitive to the needs of the population, including advanced clinical assessments, history taking, diagnosis, plan and deliver treatment, prescribe and evaluate. Working as part of a multidisciplinary team and across the GIMS single managed service, the post holder will be responsible for co-ordinating the delivery of urgent health care needs of people with IBD with an exacerbation of their disease within the IBD Hot clinic at risk of hospitalisation.

The post holder will provide specialised in-reach to the Emergency Department to triage, assess and input as an independent practitioner to patients with an exacerbation of their IBD. The focus will be to prevent unnecessary acute hospital admissions and enable patients to remain safely and supported at home and prevent premature admissions or readmissions.

Qualifications
Essential
  • MSC in Advanced Practice
  • Relevant professional health degree
  • Specialist Practitioner
  • Evidence of recent CPD
  • Independent non-medical prescriber
  • Current registration with NMC

Desirable
  • If already an 8a ANP, previous experience of inflammatory bowel disease although the candidate will undergo and intense period of training in this area

Skills & Knowledge
Essential
  • Advanced clinical skills Able to work as an autonomous practitioner
  • Able to demonstrate care which reflects evidence based practice
  • Can demonstrate experience of acute disease management and crisis care
  • Communication skills including presentation skills
  • Leadership skills with ability to engage and deliver change through negotiation and partnership working
  • Management skills with team building abilities
  • IT skills
  • Ability to manage complexity and develop and sustain partnership working with both individuals and across organisations
  • Ability to manage high pressured situations
  • Able to prioritise workload, achieving a balance between clinical and other aspects of role

Desirable
  • Competent in databases and spread sheets

Training & Experience
Essential
  • Substantial post registration experience
  • Leadership experience at Band 7 or above
  • Proven experience of working as an independent practitioner
  • In-depth knowledge of recent NHS legislation
  • Experience of leading, planning and completing audit / research
  • Experience of teaching and mentoring of staff across the multi-disciplinary team
  • Demonstrates specialist expertise underpinned by theory acquired through CPD
  • Evidence of involvement in the development of protocols and clinical audit
