Job summary
We are looking for a compassionate and motivated professional to play a key role in our Open Access Follow-Up Service, supporting patients throughout their breast care journey. In this rewarding role, you will coordinate and manage the service for both new and existing patients, ensuring they receive timely, safe and effective care.

You will be the central point of contact for patients, handling calls related to new symptoms, concerns or reassurance needs, and triaging them appropriately. Your work will include assessing patients who require review, arranging appointments and working closely with our multi-disciplinary team to deliver seamless care.

As you develop in the role, you will progress to working as an autonomous practitioner, demonstrating specialist knowledge of breast diseases and treatments. You will carry out comprehensive assessments, plan and evaluate care, request diagnostic investigations such as annual mammography, and communicate clearly with patients to support understanding and informed decision-making.

You will also attend weekly MDT breast meetings and contribute to high-quality, patient-centred service delivery.

Main duties of the job
You will coordinate and manage the Open Access Follow-Up Service, supporting both new and existing patients within the pathway. You will act as the first point of contact, responding to patient calls regarding concerns, new symptoms, or reassurance needs, and triaging these appropriately. You will identify patients requiring review and arrange any necessary appointments. Throughout the role, you will work closely with the multidisciplinary team to ensure safe, effective and seamless care. Once competent, you will practice autonomously in managing the service and contribute to its ongoing development.

You will build expert knowledge of breast diseases and treatments, request required diagnostic investigations such as annual mammography, and communicate information clearly while checking patient understanding. You will provide education and support to patients and their families, ensuring access to appropriate resources. Attendance at weekly multidisciplinary breast meetings will form part of the role, supporting collaborative decision-making. You will work with colleagues to maintain a high-quality, patient-centred service and act as a patient advocate, helping individuals understand and make informed decisions about their care and treatment plans.

Qualifications

• NMC Pin and at least 2 years experience working in open access follow up service
• Breast care course and advanced communication

Knowledge and Experience

• Breast care experience at band 6 for minimum of 3 years in OAFU service
• advanced communication course
