Job summary
This team will provide an active involvement in the assessment, development, implementation and evaluation of service user care which focuses on ensuring that the service users are supported to remain in their own homes wherever hospitalisation can be safely and appropriately avoided. Our service will deliver home treatment 24/7 to an identified group of children and young people in Crisis by responding quickly to those referred with urgent and high-risk mental health needs, and provide collaborative, effective, evidence based interventions at the right time with the right person.

There will be scope for creativity in this team, to develop evidence based interventions, to think systemically and work collaboratively with children, young people and families, informed by the local Transformation Plan, Future in Mind and the THRIVE framework. It really is time to think differently and creatively about how we can help more children and young people with their emotional wellbeing and mental health. You will be supported by some very lovely people in all parts of our service and the wider partnership

If you are currently working at an AfC Band 5 position, you are encouraged to apply for this role as part of a development programme.

You must be a fully registered Nurse, Social Worker or Allied Health Care Professional with appropriate mental health experience to apply. Working 37.5 per week you will be required to cover a 24 hr rota on a shift basis.

Main duties of the job
  • Provide specialist assessment and care planning in conjunction with service users and carers, evaluation of individual treatment plans and discharge planning.
  • Contribution to the delivery of home treatment through nursing care to an identified community client group and within a multi-disciplinary team.
  • To provide an active involvement in the assessment, development, implementation and evaluation of service user care which focuses on ensuring that service users are supported to remain in their own homes wherever hospitalisation can be safely and appropriately avoided.
  • To contribute to the clinical service development as part of the 0-25 Emotional Health and Wellbeing Service.
  • Triage and assess referrals received by service in liaison with multi-disciplinary colleagues as appropriate. Formulate and action appropriate response based on need, risk and urgency.

Person Specification
Qualifications and Training
Essential
  • Registered mental health Nurse or Allied Health Professional on NMC or HCPC Register
  • Post-registration experience, evidence of formal courses and training at diploma/degree level
  • Conforms to NMC/HCPC requirements
Desirable
  • BSc degree in nursing/relevant professional degree or equivalent level
  • Qualification in providing supervision
  • Training or relevant experience in managing suicide risk and risk management
  • Training or a sound awareness of common physical health conditions and their relationship with mental illness and wellbeing
  • Registered Sign off mentor for nurse students (nurses only)

Experience
Essential
  • Experience with working with children & young people experiencing mental health related difficulties
  • Experience of working with people who have difficulty in engaging with services and have complex mental health needs
  • Experience of working in community setting and making decisions autonomously
  • Experience of assessing, planning, co-ordinating and managing people with mental health problems in a community setting
  • Experience of integrating care delivery across multi-disciplinary /multi agency teams
  • Experience of leading/contributing to CPA/Care Planning process
  • Experience of dealing with mental health related issues/referrals/need for advice by telephone including at times of high demand and in relation to highly stressful/emotive situations
  • Experience and ability to carry out clinical audit
Desirable
  • Experience of presenting at local/national level
