Job summaryMPFT are the new providers for the Health and Justice Services in Sussex, formerly Forensic Health Non-Custodial Pathway.Comprised of Sussex Liaison and Diversion Service and Reconnect Sussex. We have an exciting opportunity to work with the new Health and Justice Service, as a registered practitioner with Reconnect Sussex.Health & Justice Services identify, assess, refer, signpost and support people to access and engage in services to address their health needs and help improve Criminal Justice outcomes.We welcome applications from a range of registered health professions and value those with Lived Experience of health vulnerability, criminal justice, and/or leaving care. Main duties of the jobThe post holder will work autonomously and as part of a team; confident in decision making, risk assessment and documentation as they will be responsible for assessing and addressing the health and social needs of prison leavers.They will produce holistic needs assessments in HMP Ford and the community, producing risk assessments to ensure risks are mitigated, contributing to reducing reoffending and promoting health outcomes. Cross-cover to HMP Lewes and IRC Gatwick may be expected when required. Additional vetting alongside DBS will be needed as part of the recruitment process.The post holder makes referrals and liaises with secondary care, social care, housing and third sector services, communicating effectively with a wide range of professionals, partner agencies, families, and carers. support plans will be co-produced with the leaver and partner agencies avoiding duplication and miscommunication.The post holder will support Health Navigators, Support Time Recovery Workers, and Peer Support Workers in the team who work with the leavers in the community for up to six months. They will help manage crises, adapt to challenges, barriers and help the leaver problem solve. They will be accountable to the Team Leader and on occasions will assist other parts of the Health and Justice service.

Identify and highlight to the Team Leader relevant issues in relation to the needs for people and their carers; assessed using best practice evidence, in order that these are reflected in service planning, development and future provision. Actively engage in the organisations clinical governance initiatives, for example to participate and lead in audit, clinical supervision and bringing critically evaluated evidence and research into practice. Participate and lead on research within area of clinical expertise. Participate in the development of the service to ensure clinical governance processes are actively implemented at team level.
Systems and equipment Maintain timely data collection and regular inputting of information through the use of data collection systems e.g., daily diaries/RIO, Safeguard, ensuring that confidentiality is maintained at all times in accordance with legislation and Trust policy. To work in partnership with other service providers to access and provide equipment and opportunities for service users to maximise functioning where possible. Responsible for ensuring actions are carried out where safety alerts are issued regarding equipment.
Person SpecificationQualificationsEssential- Registered Mental Health Nurse/ Learning Disability Nurse/Registered General Nurse on NMC Register.
- Professional Social Work qualification, DipSW, CQSW, CSS, BA, MA or Foreign qualification where certified by the Professional Social Work regulatory body. Ensuring current registration with the Professional Social Work regulatory body at all times.
Desirable- Post-registration experience, evidence of formal courses and training at diploma/degree level.
- Psychological intervention Qualification including EMDR, CBT, CBTp, Family Interventions, CAT, DBT, Structured Clinical Management
ExperienceEssential- Evidence of extensive post registration experience with working with Service Users experiencing mental health related difficulties
- Experience of working with Service Users who have difficulty in engaging with services and have complex mental health needs
- Experience of assessing, planning, co-ordinating and managing people with health problems in a community setting.
Desirable- Experience of engaging with Children and Young People and their families.
- Experience of presenting at local/national level
