Job Summary
An opportunity has arisen to recruit an Advanced Clinical Practitioner within our Integrated Mental Health Teams based in Burton on Trent.

We are seeking dynamic, highly motivated Advanced Clinical Practitioner ideally with experience of working in a community setting. As a registered professional, you will be committed to providing a high-quality service.

The Integrated Mental Health Team is an innovative secondary care community team that is guided by multidisciplinary collective leadership, providing evidence-based psychological, occupational and medical interventions to patients with a wide range of complex mental health needs.

The patient's journey is guided by psychological formulation of his or her presenting needs and difficulties, which subsequently inform the development of a plan of care for the patient. We draw from evidence-based interventions to provide high quality and effective individual and workshop treatments.

The usual hours of work are Monday to Friday 9am - 5pm. This role will require you to travel over a large geographical area, so you will need suitable access to transport to meet this requirement.

Main duties of the job
  • Providing high-quality evidence based person-centred physical and mental health care to service users within the adult community neighbourhood team.
  • Providing leadership in the delivery of high quality, safe and effective care to the wider multi-disciplinary team.
  • Contribute to service innovation and improvement, ensuring excellence in community care delivery.
  • Facilitating learning through the development and delivery of specialist clinically focussed education and training programmes.
  • Support, plan and coordinate the physical healthcare needs of service users in the community service; including the requirement for annual physical examinations and investigations.
  • Order necessary investigations, and provide treatment and care both individually, as part of a team, and through liaison with / referral to other agencies, e.g. ECGs.

Person Specification
Qualifications
Essential
  • RMN Active on NMC
  • Advanced Clinical Practice MSc
  • Independent prescribing qualification

Desirable
  • Training in venepuncture and ECG recordings (post holder will be required to complete this training within 3 months if not already held)
  • Therapeutic psychological interventions qualification

Experience
Essential
  • Evidence of working effectively at a senior level in a community multi-disciplinary health and social care setting (mental health)
  • Experience of working with a range of statutory and non-statutory agencies
  • Evidence of promoting research, evidence based practice, clinical excellence and quality approaches to support outcome focussed care

Skills
Essential
  • Knowledge of various approaches to mental health assessment Awareness of basic research methodology
  • Skills in assessing, planning and evaluating mental health care/treatment interventions
  • Skills in co-ordinating complex care and ability to coordinate and deliver teaching programs and skills in teaching and assessing/working independently.
  • An ability to co-ordinate, develop and deliver a program of health promotion interventions to patients

Desirable
  • An ability to deal with challenging situations
